Sensitive, sore nipples are usually a sign of gynecomastia. In simple words, breast tissue normally seen in women, appears in male breasts. This process can be caused by testis cancer (can be the first sign in 10% of testis cancers), hormonal issues, liver failure, some medicines.

I suggest you to start with breasts ultrasound. If gynecomastia is confirmed, you should consult endocrynologist and consider testis ultrasound.
