My wife is suffering from severe/extreme pain in lower abdomen during menstrual cycles.

Kindly provide some more details:
1. Does her period come monthly(regular)?
2. Does she has normal flow or heavy bleeding?
3. Does she has pain during intercourse?
4. Is she having any other symptoms, other than pain?
5. Any Excessive or prolonged vaginal discharge?
6. On what basis her doctor has diagnosed her with adenomyosis? Has she gone for abdominal/pelvic ultrasound or any other imaging investigations?

The details will help me to reach to a better conclusion.

Sometimes normal sometimes heavy(not very heavy). With cramps.
3. Does she has pain during intercourse?
No

Hello,

Sorry for late response.

With this details I can say that she may have fibroid uterus or adenomysis.

As you told that your doctor is suspecting adenomysis on the basis of symptoms and ultrasound,you can further confirm it by pelvic MRI.
I would also like to review her ultrasound report. (kindly upload here, if possible).

The abdominal pain in case of your wife is not responding to contraceptive pills.
In such cases, levonorgestrel-releasing Intra uterine device can be used. Other than this, aromatase inhibitors like letrozole can be used.(prescription drug)

However, the only definitive cure for adenomyosis is a hysterectomy, or the removal of the uterus.

If the symptoms does not resolve with the drug therapies and your family is complete, then hysterectomy can be considered.

Kindly discuss this with your gynecologist.

Thank you mam for your answer . Clarification required. So in one of the reports it says :
Direct Smear Occasional pus cells, epithelial cells and bacteria seen.
What is this related to then ? to some bacterial infection or its because of adenomyosis ? And also the blood count seems to be less it has been flagged red in some of the reports any reasons for them ?

The important thing in culture examination is that there was no growth of bacteria after 48 hrs of incubation. This signifies that there is no infection.
Occasional bacteria is not a worrisome issue.
It is not because of adenomyosis.

Similarly, the values flagged in red are just slightly below the normal range.
This is also not significant.

In my opinion, these are normal findings and you should not worry about it.
