Suggest Remedy For Severe Pain After Tooth Extraction

Question: i have written questions about my mouth problems recently. I was wondering if i could have osteomyelitis where 3 teeth were extracted 3 weeks ago' i press on bone above extraction and it hurts. hurts to the back of the gum. i've been on clindamycin for 31/2 weeks. my last dose is tonight. i also haveear pain. i had fibroma removed inside corner of mouth 2 weeks ago not healing well. i'm real scared. my dentist doesn't seem concerned. He did remove a bone chip 11/2 weeks ago be cause of my same complaints. He says he didn't see any infection. did dental x-ray . can you see infection on digital dental x-rays. i have an appointment tuesday. should i wait for appointment or go to e.r and request ct scan. also is a dmd like a oral surgeon. what is the difference. he has dmd and dds after his name

Thanks for your query. I have gone through your query.
As far as the pain in the bone where the teeth have been removed, I have the following possibilities.
It can be because of a bony spicule present at the site of extraction, that might result in severe pain. The other possible cause can be a osteomyelitis, but in this condition the pain will be dull and associated with pus discharge, fever and body pain.
I advise you to consult a oral maxillofacial surgeon or a person who is specialised in surgical procedures(its same as DDS or DMD). You need to get a CT done to confirm the diagnosis. If you have an appointment then wait for another day. Otherwise you can go to ER and take an opinion. Do not take antibiotics for more than 7days, because you will develop resistance. Get your blood investigations particularly Random blood sugar level since you are telling you have diabetes. If diabetes is there, the healing of wounds will be delayed and there are chances of the same wound getting infected.
So get the blood investigations done and CT and let me know the details of the report and later we can discuss it further.

Based on the report i can tell you the further diagnosis and treatment plan. If possible upload the images of the wound which is not healing and area where you have pain. I did not find any X-rays uploaded in this post. If you have the X-ray, then please upload it again.
