Question : Hello, I have taken oxycontin for several years for my chronic pain. I recently started taking pantoprazole twice a day over the last several weeks. My pain has escalated a lot for no apparent reason to the point where I can hardly function and can't think of anything else but my pain. I stopped taking the reflux medication and realized my pain has decreased a great deal. I am wondering with the special coating that is on the oxycontin if the reduction in acids in my stomach from using the pantaprozole has been preventing the oxyxontin from getting into my system or breaking down enough to pass through the stomach wall. It just seems to make sense that from stopping the pantaprozole my pain is managed to a livable degree.
Brief Answer:
I will need some information from you.
Detailed Answer:
Hi,
Welcome to Health Care Magic, Thanks for writing to us,
I am Dr.Muhammad Ahmad, I have read your question, I understand your concern and will try to help you in the best possible way.
I would like you to provide me will names of ALL the new medicines you started. After which your pain got escalated.
Only after that i will be able to come up with an answer.
Because coating is normally not the reason of drug malfunction mostly it is explained by a drug interaction.
I am waiting for your worthy input.
Regards,
Dr.Muhammad Ahmad.

the only other new medication would be prozac
I have read that any proton pump inhibitors can prevent essential nutrients to pass through the stomach wall due to the lack of needed acidity to breakdown certain nutrients so my thoughts were where the oxy is time released and has that special coating it may not all be getting into my bloodstream and thus being not as effective as it should.

Brief Answer:
Enteric coating, stomach acid and prozac
Detailed Answer:
Hi,
Thanks for writing back.
Now here we have two situations, I will address them both :
1) Enteric coating:
Enteric coating on drugs enable them to pass safely from stomach acid and reach intestine intact and then get absorbed, this can either be to ave stomach , or to save the drug from stomach.
Some times it is also used because some drugs give better relief if they are broken in the intestine instead of sTomach.
This clever science relies on the fact that the stomach is acid and the intestines, where food goes after the stomach, are not. The coating is designed to hold together in acid conditions and break down in non-acid conditions and therefore release the drug in the intestines.
Now if some drug decreases stomach acidity too much, like a more than required dose of Pantoprazole, then enteric coating will not detect stomach acid and will break within stomach just like it would do in acid free intestine.
And this might effect the relief provided by the drug specially if it were destined to be broken in the intestine.
There is no interaction of absorption alterations by PPI'S other than what i have told you, so might want to adjust your PPI dose if you have reflux issues, plus you might want other lifestyle and die moDifications which will help you control reFlux with minimal use of PPI.
CAUTION>>DRUG INTERACTION WITH PROZAC(Fluoxetiene):
Oxycodone SHOULD NOT be used with Fluoxeteine.
Using Oxycodone together with Fluoxetine (Prozac) can increase the risk of a rare but serious condition called the serotonin syndrome,
which may include symptoms such as confusion, hallucination, seizure, extreme changes in blood pressure, increased heart rate, fever, excessive sweating, shivering or shaking, blurred vision, muscle spasm or stiffness, tremor, incoordination, stomach cramp, nausea, vomiting, and diarrhoea.
Severe cases may result in coma and even death.
You should contact your doctor immediately if you experience these symptoms during treatment. In addition, combining these medications may increase side effects such as dizziness, drowsiness, and difficulty concentrating. Some people may also experience some impairment in thinking and judgement.
I would suggest you to get an alternative of Prozac.
I Hope this answers your question, if your have more queries feel free to ask, otherwise close the discussion and rate it. Wish you the best of health.

Dr.Muhammad Ahmad.
