Question: What could be this on the lower back of a 45 year old male

With the picture it appears to be a large pedunculated skin tag with the shiny skin over it. It may be infected if you have pain. You do not have to worry about it as it is very common to find in obese people. It is benign and not harmful . Is it painful ? If so it is infected. You can take normal pain killers for the pain to subside.
You may get it removed by a surgeon or Dermatologist.

It's not at all painful so we are alarmed that it mite be malignant.

YOu can choose to wait and watch it. It does not turn malignant if it is acrochordon. YOu can see a Dermatologist to confirm it. He can remove it and send it to pathology as well. If you fear about malignancy you better get it excised and examined.

What are the symptoms of malignancy also it is not painful however it seems to ve increased in size over time

Just increase in size does not indicate malignancy, I suggest a pathology examination after biopsy the lesion. Your surgeon should do this job and send for HPE. Once the report is there you can be more confident about its benign nature.
