Suggest Remedy For Sleepiness While On Medication For Anxiety

Question: hi



i am being treated with anxiety disorder(OCD) since last one and half year.



in early days of treatment i used sleep a lot because of medicines.

and now it has become my habits even if i dont take medicine in night.



the problem is i am not able to wake up in morning. even if i wake up i sleep again thinking will get up after some time. i just go on postponing it. and its not just the case with awakening, i also have habit of postponing my tasks.



i have become a bit careless may be because of anti depressents i was taking.



i also used to be lazy.



how i can overcome this habit?

Thanks for writing to us.

You have been on treatment since one and half year for OCD. You are taking paroxetine CR 12.5 currently (when and frequency is not mentioned).

- Please let me know at which time do you take pexep cr 12.5?

Two possibilities are there for your laziness.

1. It is due to depression secondary to OCD or depression itself is there.

2. Side effects of medicines.



In first case detail history is needed to check that there is depression or not. More sleep and avoiding tasks are symptoms of depression. In that case with medicines cognitive behaviour therapy(CBT) will be helpful. For excessive sleep modafinil or armodafinil can be given with antidepressant like pexep.
This laziness is likely due to learned helplessness. In which person does not try to come out of the situation due to defective cognition. In CBT psychiatrist/psychologist helps person to identify his/her automatic thoughts and faulty behavior which would ultimately be modified by person with active efforts and help of therapist. It also includes time management, daily sleep, meals and exercise timing.

In second case when you do not have any anxiety or OCD symptoms since long and still you are on medicine sedation and lethargy are side effects of medicine. Even if you miss the dose sometime drug is there in body consistently as it is controlled release and you are taking it since more than a year. In that case need of gradual dose reduction and stop the drug.

Positive point in your history is that you are aware about your laziness, more sleep and postponing tasks. In such case with self-motivation and/or with motivational counseling by professional can help significantly. You can take help of family member if it is available. Just ask them to wake you up at specific time and you need to go to morning walk and do exercise. For 3-4 days this will be difficult but gradually you will feel good and then you yourself will wake up and do these activities. Just try it.

I suggest reading a book 'Feeling Good' by XXXXXXX burns. It will help.

i dont feel any depression is there. i feel good , sometimes i feel less motivation but i still feels good. not depressed at all.

i take medicine in daytime only. so not sure if its much of medicine effect.

i think it is my habit issue. like i postpone to come out of bed , i also postpone many small things or tasks. think , i will do after sometime. when i need to catch a train , i like to go at few minutes before time. i feel like whats the point getting there too early. i assume that i wont get traffic and any other issue and will get on time..but sometimes i stuck because of some external issue.

and about my symptoms i dont have any symptoms now. i feel my ocd is gone. dont feel anxiety.

In my opinion you should take the medicine after dinner for a while. I prescribe it mostly at night time.

As you do not have any symptoms of depression or OCD and your routine life though with some disturbance is going on well there is need of stopping the medicine in my opinion.

Your current problems are mostly due to learned helplessness and may be side effect of medicine.

Try to make schedule of daily routine and follow it. Regular exercise will help.

Discuss my opinion with your doctor. Also go for CBT or motivational counseling with help of professional. Behavioral modification is the main approach to come out of this habit.

does hindi translation of this is also available .

if not then is there any other book which is avaialable in hindi for the same purpose for a person coming out of depression or anxiety disorder.
