Question : Hello sir,
I want to know about a sudden death of my mother .at the time of death she felt a severe headache n with in 2min she expired. She was having headache since 1 month.taking pain killers wn she felt. Her bp ws 120 /80 usually. Somtime it came 140/90 and one month back her bp was came 170/100 but after that her bp was 120/80.......
6yr back her angiograpy ws done for tmt +ve test. But there ws no blockage.after that she ws taking amlodipine 5mg regularly plus acitrom and asprin since 4 yr. But since 1yr she ws not taking these drugs because her bp found 100/70 n feeling chakkar.. ghabrahat. Dr stopd that drugs ...
What would be the most probable cause of this sudden death...?

Post-mortem’ is the only definite way to conclude in such cases / all else is only guess work

Usual cause of sudden death is due to heart… a quarter of patients don’t have any symptom
It could be a heart attack or an arrhythmia (disorder of heart beats)

This history here is highly suggestive of stroke – most likely bleeding in the brain, from ruptured blood vessel. Headache goes with it.
History of sudden rise in blood pressure before – may mean it could have shot up this time. There are sudden outbursts of hormone in certain conditions – like adrenaline as in Pheochromocytoma – shooting of blood pressure, heart rate, feeling of fear, tremor and so on.

Positive TMT means possible atherosclerosis – no blockage may mean normal / spasm / or it is small vessel disease and diffuse, that could not be treated by surgery or stent.
There could have been similar changes in brain blood vessels / again asymptomatic
6 years is rather long – the disease could have progressed…
