Question: Hi
Just wondering
I have ache in joints in front of my ears tender to the touch feels tight and I am getting lightheaded noughts but just come and go do not last long I have been told in the past that I suffer from tmj as well as sinisitus I just want to make sure not cardiac related
Any suggestions

Regarding your actual clinical symptomatology I would say it seems more likely to be related to a temporomandibular joint disorder.

A variable pain around the joint area, sometimes irradiating to the neck, shoulder, around the ear and face, exacerbated by mouth opening or closing, speaking, chewing, and even wit hlightheadedness, and dizziness.

It is quite specific for temporomandibular joint disorder to produce pain when touching over the joint area or pressing the articular neck of the jaw (mandible).

As you have exactly the above prescribed clinical complaints, temporomandibular remains the main responsible cause of your pain.

Concomitant sinusitis is not likely, in the absence of nasal discharge and associated symptomatology.

I recommend you trying any anti-inflammatory drugs, like naproxen, ibuprofen, etc. for relieving the pain and responsible inflammation.

For properly addressing TMJ disorder, I advise you to consult with a maxillofacial specialist (for exam. a dentist) for a careful physical exam, and some additional tests, like:

- a complete blood count, PCR (to confirm inflammation),
- CPK to investigate for possible jaw muscle disorder (which sometimes produce such complaints)
- X ray films for investigating better TMJ degenerative changes,
- Erythrocyte sedimentation rate to rule in/out a temporal arteritis ( reason for such complaints)
- Rheumatoid factor is rheumatoid arthritis is suspected.

At the end, I would like to assure you that your actual clinical scenario has nothing to do with your heart.

Your clinical symptomatology, your epidemiological data, and previously performed tests (from your previous medical history) suggest such a conclusion.

Sorry to bother you it seems I always have a question I seen my gp yesterday about random pains I get in my arms mostly the left arm she said it could be from my fybro she took my blood pressure it is was good 100/70 and took heart rate good as well she didn't think it was cardiac related but I would like a second opinion the pain is random comes and goes sometimes I get it in my fingers as well
Other than this I do get neck pain but it doesn't radiate just comes and goes
I do go for mesages and Physio is there something else I should be looking at

Regarding your concern your symptoms don't seem to be cardiac related.

Is the pain in the arm modulated by body movements or arm movements? This would be in favor of a musculoskeletal issue.

Pain in the arm, irradiating to the fingers raises suspicions of a nerve compression. A cervical spine disorder with a slipped disc could also cause these symptoms. The neck pain could be related to such disorder.

From the other hand fibromyalgia as you know is commonly associated with pain in different parts of the body.

I would recommend performing a cervical column X ray and a nerve conduction study to investigate for possible nerve compression.

Inflammation tests (PCR, sedimentation rate) would also help to rule in/out possible inflammation.

What are you taking for fybromyalgia?

An antidepressant like duloxetine or other SSRI (paroxetine, fluoxetine) would be helpful in this clinical situation.

Meanwhile I would recommend avoiding straining physical activity and raising heavy weight with this arm.

So that being said it could be nerve can that also cause face tingling or can that be sinuses as well I don't think it is anything cardiac or stroke but I am wondering how to go about this I went to my Physio yesterday and he asked me if I had headaches and pain I said yes and he also asked about carpal tunnel the tingling in the face came lasted about afew seconds and now just around my eye almost like a burning sensation like I scratched the skin
I know my blood pressure is good I wonder if this can be coming from the neck or sinuses or even tmj
Any concerns or suggestions my nose has been abit runny today as well as my eyes have been watery

Regarding your concern, I would explain that headaches, face tingling or arm pain could be explained by fibromyalgia.

Nevertheless a sinus infection can not be excluded. The typical symptoms relate to a sinus infection would be headache and pressure mostly behind the eyes, which is aggravated by head bending, coupled with running nose.
Your actual symptoms (running nose) could be related to an allergic reaction, but could also be complicated to a sinus infection (fungal, bacterial). A sinus X ray and a sinus discharge culture would help rule out this possibility.

Regarding the carpal tunnel syndrome, a nerve conduction study as I mentioned before, would be very helpful to investigate for this problem too.

Again, I would reassure you that your symptoms are not cardiac related and other causes should be investigated.

I hope ur well I do have one more question even though I did all these tests in the past and everything came back normal can you tell me then I have to c my gp this week and I have had this muscle like spasm in left chest comes and goes no pain and does not last long it does not feel cardiac but I would like ur opinion it feels like a spasm under left breast closer to breast than rib what tests should I be asking for I just want to make sure nothing is missed

You should know that these symptoms seem to be muscular related, which means that as long that this muscular contraction is always in this region and not in other parts of the body, we can exclude possible metabolic causes (like electrolyte imbalance, myoclonus, etc.).

Inflammation in this region could be related to this issue.

I recommend performing a chest X ray study and inflammation tests (PCR, sedimentation rate), to exclude possible inflammation.

A local myorelaxant ointment could be helpful during these episodes.

Thank you I will ask for xray right now I feel my heart rate beating fast that could be maybe anxiety and with that other symptoms are coming on arm pain that I have had before
As long as u don't see any of these symptoms as cardiac pain the arm is hurting from thumb to wrist then just in one spot in upper arm comes and goes
Not chest pain no shortness of breath but I just want to make sure I am ok to wait to see my dr tomorrow
