Question: I experience severe urge to scratch my anal area at night. Pics attached. Cannot see worms. Please help.

Hello. Welcome to Healthcaremagic.com. I have read your query. I have seen the attached photographs also. I understand your concern.
As per details, the most likely possibility for your symptoms is a condition of skin called Tinea cruris. It is fungal infection of skin involving the body folds. It is characterised by well defined red colored or hyperpigmented rash associated with itch. Various precipitating factors for the condition are excess sweating, hot humid weather. It is more common in Diabetics.
In such cases, I prefer to advise topical antifungals like clotrimazole cream / miconazole cream to my patients. Oral antifungals like fluconazole / terbinafine / itraconazole also help. You should keep the area clean and dry.
Visit your doctor so that he can examine you and give you prescription. He may order KoH test foryou to look for fungal elements. You share my opinion with him. I hope it helps.

Thank you for your reply. I have the following creams:
1. Ketoconazole 2%
2. Mometasone Furoate and Clotrimazole
3. mometasone Furoate

I could buy Terbinafine if you recommen that over the above. d

If only cream doesn't work, you may need oral antifungals also.
