Brief Answer:
Use antibiotics, potent pain killers and proceed with MRI

Detailed Answer:
Hi! Welcome to health care magic!

Based on the history you have shared, it seems as if you have already been through alot already. It is sad to know that you had to experience all this. To me it looks like some infection from tooth root extending into skull bone and extending pressure symptoms on sensory nerve supply of the area.

If CT scan has failed to show anything, MRI will be next best step in such circumstances. I wonder have you been prescribed with prophylactic antibiotic cover already or not, because as your history is suggestive, the extension of the infection looks to be the primary cause. Using antibiotics like augmentin or cephalosporins might have helped with gram positive bacteria which are usual culprits in such infections alongwith cover for atypicals with metronidazole.

Pain killers that you have used, are quite potent but if they are not effective with the pain management, next best ones can be oxycodone or tramadol. You can also use fluoxetine or carbamezapines along with these.

Once the extent of infection or damage is localised by the MRI, the right nerve to put block into can be identified. You need to keep close liason with ENT specialist, orthodontitian and anesthetist for the complete management.

Hoping you a speedy recovery. If you dont have any more questions, please close the discussion. You can also reach me directly through my profile in future, in case of any more questions. Regards.

Brief Answer:
You can discuss with your GP for prescription

Detailed Answer:
Hi! Like I told you, yours is a multi disciplinary issue now and encompasses ENT, orthodontitian and anesthetist so till your MRI and other work up you can start prophylactic antibiotics after discussing wih your GP or your dentist as these may not be OTC drugs and you will need prescription for these. If you dont have any more questions, kindly close the discussion. Regards.
