Brief Answer:
Go for RNA PCR test

Detailed Answer:
Hello,

Welcome and thanks for posting your query to the forum.

I can understand your concern for HIV after the latex rupture during intercourse.

As I can get you had intercourse on 2nd May 2014 and gone for Tridot test at 20th May. According to this the day of test is 18th day since the intercourse.

In my opinion it is not the correct time to go for Tridot test. It is basically antibody based test and we generally go for one between 6-8 weeks and the other at 12th week. So any antibodies based test before 6th week is inconclusive. It is because of the fact that the antibodies to HIV virus if present are not detectable before latest of 6 weeks.

At this time I suggest you to go for RNA PCR test. It detects directly the viral copies if present. So for the best possible test at around 18 days of probable exposure you can go for RNA PCR. It is availale in all good labs at XXXXXXX and accoridingly you can go fo rthe same.

To tell you more about your incidence- In my academic experience with your kind of experience of condom rupture, there is least of chance of HIV infection. For the best of relaxation of your mind let em tell you that in my clinical experience also I have not found anybody positive after condom rupture.

So relax yourself and get tested for RNA PCR to relive all your anxieties.

I hope these information will help you.

Brief Answer:
RNA PCR at 6 weeks is conclusive

Detailed Answer:
Hello Young man,

Welcome again to the forum and thanks for your swift follow up.

Regarding your further queries-

- You can go for the RNA PCR testing at any good reputed labs (like SRL or ranbaxy or related lab). Though some of the expert goes for antibody test at 12 weeks for further reconfirmation but I consider the test to be conclusive and there is no need to go for any further testing. The cost of the test is around Rs 3000.

The rest of the symptoms are not specific of HIV and therefore do not get panicky. I have seen people taking lot stress of HIV start feeling same feeling as written on net.

So try to relax yourself. As I said earlier, there is unlikely chance of acquiring HIV after condom break. SO do not worry. DO give me a follow up of your negative report.

My whole hearted wishes for your good health.

Hope to have provided you best information regarding your query. If you have any further queries I will be glad to help or if not then can close the query and rate the discussion.

Hi Doctor ,
according to ur suggestion I asked a test for Rna Pcr , But they don't have , SRL suggest me to take a test for DNA PCR (Qualitative) , they also suggest me for a Early Screening (p24 antigen + HIV antibody/Duo combo)
I did the test for HIV Early Screening (p24 antigen + HIV antibody/Duo combo)last day (31 days after a possible exposure , got the result negative .
Test method appear in the report as bellow :
HIV EARLY SCREEN - Microparticle Enzyme Immuno- Assay /Elecctrochemiluminescence /Chemilumimiscence Microparticle Immuno Assay(CMIA)
So I want to know is it proper test done in a 4 week mark (after 31 days ) or I need to be test again in between 06 week to 09 week , if yes which test you suggest or i have to wait for a compilation of after 84 days , another question if there is a test negative found in 31 days should it be positive after 3 months(84days ) , kind answer ...... , I just not keep my mind concentrate because of this ... , Thanks In advance .

Brief Answer:
p24 antigen + HIV antibody is very accurate test

Detailed Answer:
Hello young fellow,

Welcome again to the forum and thanks for further informations and queries.

As per your information- HIV Early Screening (p24 antigen + HIV antibody/Duo combo) is negative at 31 days after a possible exposure. It’s good news for you that the test is negative and in my clinical experience the further of tests would also be negative.

Though going by the protocol you may require antibody test at 3 months period but in my clinical experience I have observed that this p24 antigen + HIV antibody/Duo combo is a very sensitive test and especially the p24 antigen picks up the infection at 21st day onwards. So you can relax for the part of any possible infection.

But as I said for the part of guidelines and protocol you might be asked to do the tests at 3 months (which would be cent percent negative).

I suggest you to come out of the aura of HI infection. Do not further give a second thought. I reassure you fully that based on your kind of exposure as well as the results of the tests you are completely free form HIV infection.

I will be glad to answer any further queries or if not then can close the discussion and rate the answer.

Brief Answer:
Go for RNA PCR and relax your mind

Detailed Answer:
Hello young fellow,

Welcome again to the forum and thanks for your further follow up.
Now since the beginning of our discussion I am guiding you that in your type of exposure there is least likely/ No chance of HIV infection.

But as I counsel your type of situations then the person do not believe till he/she has a full proof negative report in their hand.

Now for the same you have two options:

1.     To go for RNA PCR at any time from now. As I said it can be done any time after 14 days and is one of the gold standard test for detection of early HIV infection. Today is around 35 days of probable exposure so you can very well go for the same. As I said in my experience it will be negative. It will not only give you peace of mind but shut down all the issues related to HIV as we are detecting the virus directly. There is no need to go for any further testing after RNA PCR.

2.     The next option is detection of antibodies. As per guidelines and protocol you have to wait till 3 months, that means around 2nd August. I think till that time you will think so much about HIV and related problems that it will not only hamper your daily life but also a continuous blow on emotional life.

In my patients with your situation and time I definitely prefer for RNA PCR. I suggest you to go for the same only for the sake of your relaxation. Around 5K is not a big amount but a negative report will give you most ease which you were searching from last 1 month.

I hope these informations will help you.

Hi good morning doctor ,
Thanks for ur continuous support , anxiety dosn't end up so as per ur suggestion could I go for a antibody test on 03 month mark to get a negative report . Is that ok , if so I could wait till that time , & if I go for the RNA PCR test that last mentioned method is a same test or any other test , as per ur clinical experience you always relax me from anxiety so if not necessary to take a RNA PCR test then it's fine , I will take a antibody test in 03 month mark , but I am just afraid because of that in between this test & 03 month may result change .
for ur information , from the last exposure I didn't do the physically relation with anybody & assure u never be do on future , so this test p24+ hiv antibody (negative)test result to be same if I also go for the only antibody test on 03 month mark , if so then I will go for the antibody test on 03 month mark & result to be conclusive what ever it may be , so kind suggest me... , sorry for disturbing you ..... Where ever you always suggest me not to be worry every thing is fine in ur case but proper fact is that a bad student feel always guilty in front of a good teacher , kind suggest me that the RNA PCR test is must necessary or not in my case . Thank you ...

Brief Answer:
You can wait and go for antibody test in August

Detailed Answer:
Hello young buddy,

Thanks for your follow up.

I can understand your part of anxiety and related stress you are undergoing thorough.

If you can wait for three months then there is no issue regarding testing immediately. YOU can very well wait till 2nd August and there is no hurry. As I said in my opinion you are free form HIV. It is only to relieve your part of anxiety at the earliest possible.

SO if you can just relax yourself for next 2 months period then there is no point in going for RNA PCR test. You can wait till 2nd August and can go for simple antibody test which in my opinion would be cent percent negative.

You are like my young buddy and it is quite usual to fulfill our natural desires. SO do not feel so much guilt. Any one can do such thing and therefore come out of the feeling of guilt.

DO not sit alone and keep yourself busy. Read good books or watch good program on TV and please do not read anything related to HIV (I humbly request for same).

I will be glad to answer any further queries or if not then can close the discussion and rate the answer.
