Question: Question 1 ) IBS or IBD
I am diagnosed as an sufferer of colon disease. Dont know if it is IBS or IBD. My symptoms are I can eat spicy food. If I eat it. next day I have to rush to the toilet, I have fast and large quantity of stools. Later my stomach is sick. My diagnosis was based on symptoms. I don't have any symptoms related to inflammatory disease like fever or pain. There was a blood test related to inflammation ( c reactive protein) the value was 0.23. It there any other way to rule out IBD? In case if I have ibd what are the ways to find it? I am wary of a colonoscopy.
Is there a way to diagnose if it is IBS or IBD/

Question 2 ) Exercise
. I started to jog in the treadmill in the evening. I am noticing my stomach is getting disturbed (stools not properly formed). Is this related to my weak stomach? How do I overcome this. I read some articles online that the blood is diverted from my stomach and my leg which causes this. Does this get better? How do I deal with it? Is strenous jogging bad for ibs / ibd sufferers?

Question # 3 ) Pain in the anus.
I am having pain in the anus from yesterday. The pain was a little high.
I didnt see any blood in the toliet bowl. If I feel the area with my fingers, There is an small bulge in the area of pain. What is it ? Do I need to be concerned.
Sometimes earlier i get a burning sensation after eating spicy food. It is bearable and diapears.
Are there some foods I need to avoid to make the pain comedown.
Are there oinment I can apply ?
Any medicines to try ?

Brief Answer:
This look simple indigestion

Detailed Answer:
Hello,
1) Generally C-reactive protein will be high in inflammatory bowel disease. Normal value indicate less possibility of disease but can not deny completely.
2) In inflammatory bowel disease you will have mucus discharge along with stool + increased frequency of stool + liquid consistency of stool + lower abdominal vague pain. Also you might have weigh loss because absorption of nutrient from diet will be affected.
3) In IBS, you have irritation for particular food or may be your digestion would be improper. This lead to diarrhea after ingestion of that particular food. But sometimes you may get constipation also in IBS (constipation type). Above mentioned other symptoms would not be present in IBS.
4) There is neither correlation nor harm of strenuous exercise on symptoms of IBS or IBD. You can continue cardiac exercises (aerobics).
5) Your anus problem is external hemorrhoid with possibility of anal fissure.
Feeling of something bulging is from hemorrhoid (It can be seen in picture) and
burning sensation is due to anal fissure. You can use local application of Lignocaine for burning sensation at anus. Try to prevent constipation and for this Isabgol is effective but you must take it once evaluation will be done for IBD/IBS as it may worsen diarrhea otherwise.
6) You must undergo colonoscopy as this is confirmative investigation for any pathology of intestine.
7) Your symptoms neither correlate with IBD nor with IBS, it look like due to simple indigestion.

I went to the doctor today and I was diagnosed with Prolapsed Anal Hemorrhage.
He has suggested stool softner and a cream to apply
Have the following questions, How long does it take before it gets normal ?
I used to have a occasional blood stains on the tissues, Does it mean I had internal hemorrhoid ?
Are there any connections with my IBS / IBD condition?
What are the causes of this condition ? Is it related to the food I take?
What precaution for the future?

IBS vs IBD

Regarding IBD I dont have any symptoms mentioned No Mucus discharge along with stool + increased frequency of stool + liquid consistency of stool + lower abdominal vague pain.

In my condition whenever I eat spicy food ( chilli, ginger, peppper) the next day morning I have an urge to visit the toilet and it is a diarrhea.
Please see my earlier posts related to this.
I have tried taking gelusil and ranitidine. But it didnt help
I will be happy if this is an indigestion problem since It can be cured.
Pleasee suggest how I go about this.

Brief Answer:
Prolapse hemorrhoid cause bleeding. No relation with IBD/IBS

Detailed Answer:
Hello,
I apologize for late reply
1) Ideal treatment for prolapse hemorrhoid is surgery but sometimes it is not possible due to swelling from prolapse. Thus initial medical treatment is prescribed to taper down swelling and later surgery can be planned (2-3 weeks).
2) Bleeding is from large hemorrhoid due to friction with stool. It is not appropriate that if you have bleeding means you have internal hemorrhoid.
3) There is no correlation with IBD or IBS. Prolapse hemorrhoid is different condition.
4) Diarrhea after eating spicy food - suggest indigestion and avoid junk food, spicy food, oily food
5) Gelusil or Ranitidine will not help in indigestion. You might require intestinal enzyme supplementation for indigestion
6) I suggest you to take treatment for indigestion and prolapsed hemorrhoid. If your symptom of indigestion persist after treatment then go ahead with colonoscopy when swelling from prolapsed hemorrhoid come down.

Thanks Doctor for your detailed response
For prolapse hemorrhoid, Is surgery the only option? Does it cure on its own ?
I want to avoid a surgery if I can.

Few questions on this
1) What are the causes for hemorroids? Is it because of lifestyle ?
2) What should I do going forward so that I dont get it again ?
3) What are the risks I carry due to this, in terms of another prolapse again and any other risks I need to be careful of ?
4) Are there any specific food I shouldd avoid ? Some say avoid spicy food, Is it true.

For intolerance to spicy food.
I am happy Diarrhea after eating spicy food is due to indigestion.
Could you please let what enzyme I can take for it?
In that case I can take this Diarrhea after eating spicy food after a spicy food and I should be fine.
again, Thanks for all your time and help

Brief Answer:
Surgery is required

Detailed Answer:
Hello,
1) Prolapse of hemorrhoid only occur when hemorrhoid is large sized. Yes surgery is recommended in prolapse hemorrhoid
2) Hemorrhoid is due to constipation (70% contribution) and structural variation of anal canal (30% contribution).
3) Manage constipation. This is best way to prevent hemorrhoid
4) Prolapse hemorrhoid does not carry risk per se. But sometimes due to more swelling around hemorrhoid it is not possible to remove hemorrhoid completely and might require second surgery (very rare chance) and this is why surgery is delayed in prolapse hemorrhoid till swelling taper down.
5) Avoid - spicy, oily, junk food
6) You can try Tablet unienzyme. (I am sorry but i can not provide dosage information or schedule to take on this portal) But I suggest to consult your physician before starting any new medicines.

Thanks doctor for your response.
How long does it typically take for the Prolapse hemorrhoid to shrink so that I can be ready for surgery?
Also if I dont perform the surgery, Does it cause any complication or is it only the pain I need to bear?

Regarding my indigestion, I was Yalkut and VSL#3, I am assuming since both are probiotic they may not be of much help.
Another issue wanted to check, If I run on the treadmill for a longer duration I get disturbed stools, Is this a know phenomenon for some ? I read some articles that blood is diverted from the stomach to the leg which causes this. Could you please let me know if it is correct ? How do I overcome this?

Brief Answer:
Bleeding and pain, are complications of prolapsed hemorrhoid

Detailed Answer:
Hello,
1) If swelling is there then surgery can be done generally after 5-7 days (with moderate to severe swelling). Sometimes surgery can be done within 1-2 days of diagnosis if swelling is less
2) Persistent pain and increased frequency of bleeding is main complication for non-surgical management of prolapsed hemorrhoids
3) No. VSL-3 work is good as it contain probiotics. You can continue it for indigestion along with Tablet Unienzyme.
4) Concept of treadmill running-diverted blood-disturbed stool is wrong. There is no any association between running and disturbed stool. General physiology of human body (abdomen) is that running, walking keeps intestine movement good but it never affect consistency or frequency of stool.
