Question : Diabetology:Dear Doctor,
I am 75 year old. I have had type II diabetes since 2012. Eversince I am taking Glycomet 500 mg twice a day. During these 6 years I lost much of my body fat and muscle and became lean markedly , may be because of Metformin or less intake of protein . My weight is steady at 63 kgs and BMI is 21.3. I walk for 4 km every morning and I am not a sedentary type. My lipid profile is in normal levels. I had IHD in 1977 and BP in 1981.The BP is well under control and my blood glucose fasting is 105 . My PP level is 120 .Though i lost body fat and became lean , i never felt any fatigue or weakness. Taking all this into consideration, please suggest the best protein powder if at all i require to take or any other valuable advise .If you require info further kindly advise.

Hi,

I went through the details submitted. I am extremely pleased to know about your fitness. At 75 with diabetes, high blood pressure and coronary artery disease as comorbities you seem to be extremely fit. Blood pressure and blood sugars are in control and your BMI is also extremely good.

In this context, if you do not have any symptoms I don't think you need to add supplements. If at all necessary, you may take multivitamin and calcium tablets; but I do not favor the use of high protein supplements. You should continue with your current regimen of exercise and diet. Make sure your diet is balanced with lots of fibers, whole grains and vegetables. That should suffice.

Dear Doctor, I thank you very much for your prompt reply.I have forgotten to mention about my thyroid level. T3 0.9 T4 6.8 and TSH 4.8. I am told that i have hypothyroid. Is it true? Does this thyroid condition has any bearing on my fat loss? Kindly advise. I never felt any symptoms of thyroid either before starting thyronorm 75 mcg or afterwards. By chance i have under gone the blood test.Then it was found.

first thyroid test performed on 5.8.2011. The level is then 10.1.My physician prescribed 25 mcg first.

Sir, since the level of tsh has gone up from 1.2 to 4.8 shall i increase the dosage.Kindly advise. Is this hypo or hyper ?My doc did not throw any light on this. So far in these 7 years i did not have any symptoms or what so ever.

Hi,

Subclinical hypothyroidism is a condition where hypothyroidism doesn't produce clinical symptoms. The fact that TSH values are high and gets under control after starting thyroid supplements is the evidence that you indeed suffer from hypothyroid state. But yes, as you pointed out they are not causing any symptoms. However you need to continue thyroid supplements.

I will wish to see free T3 and T4 values before making changes to your thyroid dosage. Please discuss it with your treating doctor and if deemed right please get it checked.
