Brief Answer:
I would also suggest you to get vitamin B 12

Detailed Answer:
Hello, I have studied your case and I can assure you that you need vitamin D 3. You should take 60000 unit per week for six weeks and then check again. I think this therapy will correct your vitamin D 3 level.
After that you need to take 1000 mg calcium daily.
As far as your second question is concerned I would like to suggest you to get serum vitamin B 12 testing. And if it is low then you need to take B 12 also.
I hope you do not smoke and your blood sugar is under control.
Thanks
