Question: Hi doctor - I am on mirtaz 15 and olimelt 1/2 of 2.5 ( it was reduced from 2.5 in my last meeting with the doctor on 9th Feb), librax mor/eve for a week. In my meeting with the doctor on 9th, he added levosulpride mps in the night...I have following queries:
Librax I added on my own as I suffered a major bout of IBS for about two months, since it started giving me constipation, my doctor added levosulpride mps in the night. I am better with the IBS but constipation still bothers me, can I cut down my librax in the night, will it better my constipation? Since I have been taking it for a week, will stopping night dose give me any withdrawal symptoms?
Does levosulpride help with anxiety, is it effective for IBS, if yes, can I increase its dosage? Is it safe to take in the long run? I suffer from anticipatory anxiety, had three anxiety attacks in the past, have phobia of meetings, SSRIs just don't suit me, therefore my doc has added mirtaz and olimelt? Have gained weight by 3.5 kilos, therefore, my doc reduced olimelt to half. Do mirtaz or librax or levosulpride also cause weight gain? Heard mirtaz adding to weight, does it cause same weight gain as olimelt? Pls advice.

- Yes stopping Librax in evening should reduce the constipation. Skipping of evening dose would not cause withdrawal symptoms because you are taking the drug for a week only.

- Yes Levosulpiride helps with anxiety also. The drug is an improved version of antipsychotic amisulpiride. The drug in low doses helps to improve dopaminergic function and thus result in improvement of mood, anxiety etc. The drug helps in bowel symptoms also. The dose should be increased after a week of therapy only.

- Yes the drug is absolutely safe to take in long run. No serious side effects are there. No risk of dependence.

- Mirtazapine also causes weight gain but the gain is not severe as Olanzapine. Still the risk of weight gain is high with mirtazapine. Librax doesn't cause much weight gain. Levosulpiride also causes weight gain but the weight gain is not severe. You can take the drug without any worries. Please wait for few days to see effects of the drug.
