Suggest Treatment For Cluster B Personality Disorder

Question: Hi,
I would like to ask question related to my 65 year old mother. She is sometimes very caring but sometimes she gets super angry and says very hurtful things. I am the only child (she had total 4 with 3 kids dead - 1 as recently as 7 years ago). She thinks she is always right and never admits mistakes and is always in conflict with 1 person or other.

She also thinks very highly of her uncles and sisters but all other people are looked down upon - including my wife (which causes conflicts). She is a heart patient, blood pressure, psorisis, asthma, border line diabetes.

Are these symptoms of any mental issue?

Hi

This problem is faced by our family since atleast 25 years. According to my dad this started after giving birth to her first child - who passed aways 7 years ago.

This conflict is not just my wife, it is with my dad's sisters, her friends aswell (it starts from a innate superiority complex - driven from her family (boasting about her uncles, aunties, grand parents, sisters, brothers, parents).

She does sometimes admits but it is more like she is cornered and she says those words. Her feedback mechanism to avoid the same issue does not work it seems. She says hurtful things - when questioned she would first deny it, than get angry and start accusing people of what ever her fear is. She does not back down.

She functions perfectly. No detoriation in any function. She does get tired more frequently and once she is tired her anger increases and lashes out at even small things.

She sometimes forgets things - like calling someone etc. But not complete loss of memory. Her memory is good.

Her sleep patterns are bad. She is not able to sleep well wakes up early morning or sleeps late. Sometimes sleeps at odd hours. Total 5-6 hours sleep in a day. Her apetite is ok - no change.

Asper the provided details I feel there is enough factors indicating toward mental illness.


I feel she is suffering with Cluster B personality disorder which is characterized by dramatic pattern of behavior and impulsive nature. Among the cluster there is most likely hood of narcissistic personality disorder.

In recent past she may have developed depression which is not compatible for her personality traits and she may be behaving abnormally due to depression which is common with cluster personality disorder.

I must acknowledge that the opinion about her mental state is tentative and need to confirm with in person consultation which needs talking with patient for a length.

What is the medication that she can take for these two issues? This is not for prescriptive purposes rather to gauge the effectiveness of any future treatment.

For the depression SSRI group of medications can be used which includes sertraline, fluoxetine, escitalopram. The other option is SNRI which includes desvenlafaxine, venlafaxine and duloxetine.

For the personality part the effective medications include mood stabilizers, antipsychotics as well as antidepressants.

Which are the things that we need to look out for depression?
