Heart Burn: Treatment Options and Management
Detailed Answer:
Dear
Thanks for contacting Ayurveda with your health concern.
1. GERD [gastro esophageal reflux disease] is a common symptom today and is primarily attributed as lifestyle disorder secondary to underlying disease, so let me know about following particulars [from your side]
i. Lifestyle including
. Occupation [poor posture],
. Weight [obesity],
. Addiction [coffee/alcohol/ smoking],
. Diet: fatty and fried foods, chocolate, garlic and onions, acidic foods\citric fruits, spicy foods, eating quickly or soon before bedtime
ii. Use of any medications like: bronchodilators [for lung disease], calcium channel/ beta blockers [for heart condition] or any anti depressants, since they may cause or worsen GERD
iii. Since you are a diagnosed case with suitable examination thus your attending Physician must have rule out other serious condition like hiatal hernia, defective esophageal contractions.
2. Dietary Regimen:
Take: old brown rice,bitter gourd,white gourd melon,green vegetables,cold water, pipalli,harad,garlic,honey,amla,rice water gruel,barley water,milk,shatavari.
Avoid: newly harvested rice,chick pea flour,black gram,horse gram,potato,brinjal, saindhav namak, kanji,spicy food,tea/coffee,alcohol,junk food,day time sleeping, tomato,white sesame seed,curd,pain killers [NSAID's],
3. HOME REMEDIES [of your choice]
. Drink 4 spoons of aegle marmelos leaves juice for 14 days.
. try to suck a piece of clove for immediate relief.
. fresh mint juice
. banana, cucumber, water melon are natural antacids
. glass of warm water [early morning]
. coconut water [3-4 times a day]
. suck 10 gm jaggery everyday [after meals] if there is constant acidity.
. Mix
3 pinch dry ginger powder
3 pinch amla powder
3 pinch sugar and eat this mixture for 1 month
4. LIFESTYLE
. Avoid forward bending or Immediately lying down after eating, [cause of bitterness in mouth/and persistent dry cough], instead go for head high position at night [since GERD causes regurgitation/choking]
. loose excess weight
. maintain good posture. [gravity prevents reflux]
. do not eat one large meal at one time, instead take smaller, more frequent meals during the day.
5. Please revert back with above questionnaire for Treatment Aspect.

Dear
Thanks for reverting back.
1. Because there was no fixed regime of taking food, irregular habits resulted in fermentation of ingested food resulting in gas formation and stomach upset, causing stomach problems.
2. weight is certainly a contributory factor for GERD problem so as said earlier loose excess weight.
3. You can take Green tea since contains anti oxidants and if free from caffeine.
PS.
i. You may ask for 'TETLEY TEA' which is green tea with lemon/ginger/mint and is free of caffeine and is manufactured by TATA.
ii. Green tea teabags contain a lot more caffeine, try to buy loose green tea leaves.
iii. since caffeine dissolves quickly thus make sure you discard first infusion [universal precaution] in which 80% caffeine is released in water.
4. Avoid food which take more time to digest or irritate stomach mucosa like: cabbage/ sprouts, onions, asparagus, pears, apples, peaches, chewing gum, milk products, fruit juice, vinegar, alcohol, caffeine,greasy-spicy-refined foods.
5. A good Prescription for your associted symptoms is:
i. narikel khanda 20gm
avipattikar churna 3gm with honey
ii. leelavilas ras 125mg
trifla mandur 250mg
muktashukti pishti 125mg with trifla kwath (decoction) 3 times a day.
iii. You can take Draksharisht or Patoladi kwath im morning to increase your appetite and irritation symptoms.
6. You have to search in google for clinics/hospitals around XXXXXXX as I'm not familiar with Ayurveda doctors there.

Thanks for the reply, please make me clear on the foods that I should not take. Earlier physician I consulted, suggested me to take curd and butter milk and asked me to stop taking milk and its products. But in your first reply you said that I can take milk as well and asked me to stop curd??
Also, Fruit juices and apple also is a problem for digestion ?? I dont drink and smoke.
Please also let me know the timings for above suggested madicine and before food or after food. How long I need to use this medicine to get result?

Clarifications regarding Medicine
Detailed Answer:
Dear
Thanks for reverting back
1. Regarding use of MILK, there are different opinions:
i. Since milk contains calcium which causes release of acid, thus can aggravate GERD symptoms, so is told not to take.
ii. Since pH of milk is alkaline and that of GERD is Acid thus will Neutralize it, so can be given
iii. In Ayurveda we hold that Milk is Sheet [cold] in potency and will pacify Acid which is Ushna [hot] in potency, so can be given.
Conclusion: I then usually tell the patient for 'hit & trial' method so that he/she himself know what is best for him/her. If they find it productive one can have/take COLD milk.
2. CURD is not advised since it is Amla [sour], Abhishyandi [slimy], in nature with good source of calcium thus will lead to fermentation, difficult to digest and aggravation of symptoms, as such butter milk is advised.
PS. Freshly made CURD can be taken since it contains beneficial bacteria that help digestion.
3. Fruit Juices;
i. Orange/grapefruit/pineapple/tomato juice are very acidic and can cause reflux.
ii. Apple, carrot, aloe vera, cabbage juice are less acidic and cause fewer GERD symptoms.
iii. Milk shakes: due to high fat content will stimulate acid secretion, so avoid.
4. Regarding Medicine:
1. Twice daily [before meal]
2. thrice daily [after meals]
3. 4 tablespoon of arishta/kwath and 4 spoon of water [total 8 spoons], and take after breakfast and in evening.

Dear
Thanks for reverting back.
1. Since I have not examined you directly, kindly visit your nearest Ayurveda hospital/clinic for physical examination, who will determine your XXXXXXX & dosha and then will let you know about the duration.
PS. They will tell the prognosis/duration on the line of 'Udharva gat amalpitta'
2. Since there is Geographic/Climatic difference thus in your case, one has to take it regularly for 3-4 months.
