Suggest Treatment For GERD And Greenish Tinged Bowel Movement

Question: Hi,
Physical History: 30 M, Weight: 60 Kgs, Healthy.
I had some GERD symptoms, so I have been to GI two days back who prescribed me - Nexium 40 mg and Gaviscon. Since two days, I have a bowel movement which doesn't look my normal brown as it used to be. The color looked little light green yesterday and I was having green leafy vegetables. Today, it looked like yellow or green ?, am not sure. I am attaching the pics.

Questions:
1. Is this normal?.
2. What color stool does it look like?.
3. Does Gaviscon which is white in color can cause this color? or any side-effects of these medicines?
4. This is not chalky white color right which I am worried about most?
5. Should I stop these medicines?.

Thanks doctor..So, I don't have to go to doctor's clinic for follow-up right?.

I have a few more questions-
- I am finding it really hard to stop consuming tea and its triggering my gastritis. Any advice on it?
- My diagnosis on this condition was chronic mild active gastritis on biopsy done 3 years back. Does this mean I have to go with condition for a long time?.
- I always have manageable gastritis, but then I thought its better trying medication again. Long time back when I tried medications were of help, but the symptoms re-bounded as soon as I stopped it. Also, I read there are lots of side-effects of these medications. Am always confused. Do you have any advice on this?.

Hi,
The stool looks absolutely normal. There is no need to go to doctor's clinic.

You can slowly reduce the frequency of drinking tea and over a period of time you can stop it.
You can treat gastritis with Neksium once daily for 4 weeks.
Also get tested for H.pylori infection as it is one of the causes of chronic gastritis.

Thanks Doctor for reply. I had an EGD done 3 years back when they also took some cold biopsies from the stomach. Findings were below -
1. No H.Pylori.
2. Chronic mild active gastritis.
3. Small hiatal hernia.(this doctor said is insignificant).

So, my questions-
1. Is this condition going to be chronic as I am suffering from this since 4 years or so..and none of the medications provide full recovery. They just mask symptoms for sometime and then a cycle starts again.?. But all my other symptoms look sort of normal - manageable acidity, normal stool, normal appetite and weight gain.
2. Following things tend to help- eating frequently, avoid trigger foods(which is tea for me) and low-stress. Is this the protocol I should follow?.
3. Any advice on homeopathic treatments?.
4. Can this condition be genetic as my parents suffer from this?

Hi,
I would advise you to get an EGD done again to see if the lesions are persisting.
A recurrent gastritis is not equivalent to chronic gastritis.
And if it is chronic gastritis and H.pylori then the underlying cause should be found out.

1. What symptoms are you attributing to gastritis?
2. Yes follow no tea and stress relief plan.
3. Being an allopath I cannot comment on homeopathy.
4. No it is not genetic.

Am not sure you understood there -
"And if it is chronic gastritis and H.pylori then the underlying cause should be found out." -- There was no H.Pylori diagnosed in the biopsy and there was a Grade-I gastritis which according to GI is very mild.

I have been to two GI doctors here recently, both have suggested that repeat EGD is not required as there are no alarming symptoms and last EGD was unremarkable and there are no new symptoms. I am confused now. Can you help with this?.

Symptoms I am attributing to gastritis are:
1. Heartburn

Also, please suggest if there are any other things I need to change or follow?

Hi,
Sorry for the typographic error.
It should read - "And if it is chronic gastritis and NO H.pylori then the underlying cause should be found out." (No is missing there).

Heart burn is not a feature of chronic gastritis. Heartburn usually indicates gastroesophageal reflux disease (GERD). In that case you do not require a repeat EGD unless you have pain or difficulty while swallowing.

GERD can be recurrent.
in addition to medications lifestyle modifications are also important for relief.

1. C.Sompraz- L once daily before breakfast for 4 weeks
2. Mucaine 10 ml when you get heartburn

3. Avoid coffee, citrus. chocolates, carbonated beverages, tomatoes, fatty/spicy food
4. Avoid smoking and alcohol ( if you have these habits)\
5. Avoid eating full stomach. Instead eat multiple smaller meals
6. Go to bed at least 2-3 hours after food.
7.Stress relief methods can be followed if you have workplace/family related stress which may worsen your symptoms.

Thanks Doctor for the patience and reply. Anyway, I got a repeat EGD and Liver Function Test. Findings of EGD were:
1. Small hiatal hernia
2. Gastritis
3. Some erosions

All the liver functions(like SGOT,SGPT) were normal except bilirubin at 1.4(reference range unto 1.2). Doctor asked to ignore that bilirubin value as all other functions are normal and asked to take the medication for 3 months. Is this any cause of worry?.

Knowing these results now, do you suggest any change in your recommendations?

I have also attached the reports, please take a look.

Medication am offered is Ganaton Total for 3 months.

So, please answer the questions I have in the above post. Thanks a lot.

Also, as a side-note I am suffering from sneezing, cold and flu symptoms since 2 days. My eyes are normal and appetite is normal too.

I saw your reports.
You can follow the advice I have given in my previous post.
Avoid NSAID type of painkiller medications (For eg Brufen, voveran).

Bilirubin is not significantly raised and the liver enzymes are normal. So as your doctor has said, you can ignore it.
This is not a thing to worry about.
You can take Ganaton Total for 3 months. It is similar to the Sompraz-L which I have suggested. You can take any one of these two medications.

For flu like symptoms you can take Dolo 650.
