Question : Hi, I am 24yrs male male. I have had unconjugated hyperbilirubinemia since 2009. The lowest my bilirubin has been is 34umol, the highest was 94. Since then it has fluctuateds, it reduced to 45 but has increased again to 64umol. ALT/AST have always been within normal ranges and all other blood work is normal. I have had numerous ct and MRI scans over the years, as well as an ultrasound and urine/eye tests all of which have been normal. At one point however I had a peripheral blood smear which showed target cells, atypical lymphs and stomatocytes so there was some doubt about the diagnosis which was originally suspected to be Gilberts syndrome. I have had another smear since then which was normal. Hereditary cholestasis has also been mentioned. I have no symptoms other than drowsiness which has been thoroughly investigated and no obvious cause found. More recently though I have been experiencing some mild pain under the ribs, but my liver is still working normally and normal size. The Drs will not perform a liver biopsy to confirm the diagnosis as the abnormality is so mild and they cannot justify the risk to me. Should I be concerned?
Brief answer...given below
Detailed answer....
Hello,
Thanks for the query.
I have read your query and understand your concern very well.
Going by your history,I concur with the diagnosis of GILBERT's syndrome because over a period of so many years, there has not been any deterioration of other liver enzymes and except drowsiness ,you are leading a healthy life.
You need not get concerned about it as people suffering from GILBERT's syndrome live a normal healthy life and there is no treatment offered for this condition.
I hope,it helps, however you may revert to me for any further query.
Best of luck.
Thanks
