Brief Answer:
In my opinion, you should discuss this with a psychiatrist.

Detailed Answer:
Hello dear,
I have gone through your question.
I can understand your frustration of dealing with such feelings.

From the description of your problem with the feelings of unreality or living in a dream-like state, I can deduce that probably in this moment you may suffer from severe anxiety.

One of the most unpleasant symptoms of anxiety is the very surreal feeling of not really being part of the world anymore or also called derealisation.

So, I think that your overwhelming feeling of living in a dream all day long, can be attributed to your anxiety.

I'm curious to know your actual thyroid function tests (TSH,free T3 and free T4)because (you mention that you do not take levothyroxine at this time and for this reason, maybe you actually have hypothyroidism and this condition can cause anxiety.(let me know your results and then we can discuss if your thyroid condition s the culprit of your symptoms and about the right treatment).

So my advice for you is to consult a local psychiatrist for the right diagnosis and treatment of your anxiety.

There are also some other things which I would recommend you do, or in some cases not do, which will help in your recovery from dissociation.

- Learn how to live day by day so that you focus on what is important now and not worry about the past and future

- Exercise; It will keep your mind from worrying about how you are feeling and help give you the drive and focus required to get on with the important things in your day.

- Stop believing the things your anxiety is telling you – this is absolutely key in reducing the stress it puts on you.

- Getting a good night's sleep, this should mean at least 8 hours and with a regular sleeping pattern. Tiredness is a stress in itself and this can cause dissociation, it also reduces your ability to focus on the things you should be focusing on.

Hope my information is helpful.

Brief Answer:
Can you upload a report of your last thyrois test results?

Detailed Answer:
Hello again
Your TSH level is normal but your T3 end T4 are low.Anyway i need to know your lab normal ranges.Than we can discuss more clearly if you need thyroid hormone replacement therapy or not,in this moment.

For your anxiety and the symptom of derealisation,in my opinion you need the help of a psychiatrist as soon as possible, for the right treatement.

Hope i have been helpful.
Best regards.
