Brief Answer:
Inflammatory bowels disease can be the reason.

Detailed Answer:
Hello!

Thank you for the query.

Can you please attach a photo of it? Have you had colonoscopy ever done?

Your symptoms sound like inflammatory bowels disease (Ulcerative Colitis) with a rectal abscess caused by fistula.
All mentioned by you symptoms can be caused by this disease. You may also have bloating, abdominal pain, occasional gas obstruction.
I suggest you to consult proctologist and gastrologist. Abscess (if confirmed) should be incised and drained. Colonoscopy should be done to diagnose inflammatory bowels disease.

Hope this will help. Feel free to ask further questions.
Regards.
