Suggest Treatment For POCOS With Anxiety And Asthma

Question : I am looking for a doctor in the XXXXXXX area for my daughter-in-law. She is 30 yo and has pcos. She has seen a gyn but needs more extensive care. Her symptoms are high triglycerides, high normal fasting fsbs, extreme fatigue, night sweats, borderline Bp, snoring, rapid mood swings, had not had periods until 2012. Her mother never took to doctor. She has large waist circumference with small legs and arms she hardly ever eats and asthma. Could her rapid mood swings tie into this talking about bouts of mania then hours in bed irritability then anxiety. Should this be followed with psychiatrist and endocrinologist? Very concerned thank you.

I am Dr S Khan and i would like to answer your query for your daughter who has PCOS, Asthma, Mood swings and night sweats. 1-Lets start with PCOS. Al the symptoms of fasting blood sugar irregular menses and borderline blood pressure, increased TAGs are because of this trouble. Now PCOS needs a proper care and once managed all the metabolic symptoms will get back to normal. Losing weight and a regular exercise is the first step here.Remember diet and lifestyle modification is the first step here.Get ADA diet for diabetics. Increase omega 3 .and 9 fatty acids.Vitamin D supplements. increased fiber; decreased refined carbohydrates, trans fats, and saturated fats is necessary.Remember diet low in cholesterol and saturated fats and increasing physical activity is the key here. Medications used in the management of PCOS include the following: Oral contraceptive agents (eg, ethinyl estradiol, medroxyprogesterone) Antiandrogens (eg, spironolactone, leuprolide, finasteride) Hypoglycemic agents (eg, metformin, insulin) Selective estrogen receptor modulators (eg, clomiphene citrate) Topical hair-removal agents (eg, eflornithine) Topical acne agents (eg, benzoyl peroxide, tretinoin topical cream (0.02–0.1%)/gel (0.01–0.1%)/solution (0.05%), adapalene topical cream (0.1%)/gel (0.1%, 0.3%)/solution (0.1%), erythromycin topical 2%, clindamycin topical 1%, sodium sulfacetamide topical 10%) If the above mentioned options did not help then their are surgical options too like Electrocautery Laser drilling Multiple biopsy etc. Seek an Endocrinologist for ACTH stimulation test to rule out hormonal issues or for other causes of menstrual irregularity such as thyroid disease or pituitary adenoma. A reproductive endocrinologist should be consulted if the patient is infertile and desires pregnancy. 2-Now Mood swings: A psychiatrist is needed here. Some psychotherapy sessions with vocational recreational therapies will help. 3-Snoring issues: Once the weight is lost, snoring will vanish,Seek an otorhinolaryngologist for snoring sleep apnea.
