Suggest Treatment For Sjögren's Syndrome And Fluid In Ear
Question: HI Dr. Naval,
Well in regard to finding about whether or not I have Sjogren's as I had just seen an ENT doc, to see about fluid in my ear I don't need another referral and got in 3 weeks to get a lip biopsy on my own. Do you think he will do it without first going to see a rheumatologist?

Hey i would suggest you go for Serology first( ANA and ENA ) for the time period you are waiting. If they are inconclusive then you may need to consider Salivary Gland Biopsy. But if they come out positive and considering the symptoms you have, we dont need to go for anything invasive here. Also get a Schirmmers Test done. These are all basic tests and would give us a fair idea.
Lip Biopsy we normally reserve it for later stage if we strongly suspect Sjogrens but ANA and ENA are negative.
I would recommend you get these tests done before going for the procedure.

Okay, I will then, just heard that sometimes these test can be false positive so thought I would go for the most definite first, I will talk to my PCP.

Well, it can be false positives at times, but they do come in the diagnostic criteria for the disease. RAther if it is positive, then you dont need to go for the lip Biopsy. We always prefer a non invasive test than invasive one. But all said and done, i still feel schedule an appointment with a Rheumatologist. Examination is anyday more superior and definitive.
