Suggest Treatment For TB During Pregnancy

Question: Dear Doctor,

My wife is 20weeks pregnant and she was diagnosed with Lymph Node Tuberculosis 2weeks ago at a hospital in Beijing, CHINA. There are a few opinions from different doctors.

1. The Tuberculosis Doctor (very experienced) at the Beijing Tuberculosis Hospital said that her condition is not serious so she should not take any TB medicine until she gives birth and even after the birth the Lymph Node TB could have gone away already. He said this Lymph Node TB happened because she was wasn't feeling and eating well during her Nausea period which ended about 2 weeks ago. So if she eats well now it could increase her level of Immune system etc.

2. There are other doctors (not chinese) saying that the pregnancy is already in a safe state to take Tb medicine and actually not taking any medicine is more dangerous than taking the TB medicine (afraid of any side effects).

Starting from 3 weeks ago, her legs (lower legs not the thigh) starting hurting when she walked especially when she got up first thing in the morning. Her toes on her feet are usually cold. We had a doctors visit yesterday at a Neurologist here in Mongolia and according to her testing (with a simple tool to test her responsiveness of the nerves) she said if it continues her legs and arms might become dull and no feel at all. They did her upper body MRI yesterday and the result is coming out in a few hours. I know it is hard for you to diagnose without looking at anything but could it be that this Lymph Node TB is related to her leg pain?
What could we do if indeed her central nerve system is low?

Thank you for posting your query.

I have noted your wife's details.

Her current symptoms could be due to spread of TB to the spine/spinal cord. MRI spine would be able to confirm or exclude this.

Lymph node TB should be treated with anti-TB medications, as there is little chance of cure by self. On the other hand, the disease may spread to other parts of body.

Also, after three months of pregnancy, there is very little risk of harm to the baby due to TB drugs.

Please let me know the MRI report.

Dear Doctor,

Thank you for your reply. The TB treatment already started 3days ago. The MRI examination result came back and i will attach the summary by the doctor. If you need additional data from the CD that the hospital gave us please tell me. I dont know how to attach the pictures printed on the black film. If you need it then i will find a way to scan them.

Radiology Examination Report

Examination name: Whole Spine MRI
Technique: Whole spine MRI without contrast

Findings: Straightening of normal cervical lordosis with mild kyphotic change at C5/6 level causing partial anterior spinal canal obliteration. There is a mild signal and disc height loss of the C5/6 disc. There is normal degree of rounded curvature of the thoracic spine. Normal lordotic curvature of the lumbar spine. There is no evidence for abnormal marrow signal in whole vertebral column. There is no evidence for abnormal signal with the spinal cord. The surrounding paraspinal soft tissues are unremarkable.

Significant findings by level:

C5/6: There is a small disc bulge with mild bilateral neural foraminal stenosis. There is no nerve root compression.

Impression:
1. Straightening and mild kyphotic change of the cervical spine at C5/6 level with mild compression of anterior spinal canal.
2. Small symmetrical disc bulge at C5/6 with mild bilateral foramina stenosis.
3. Otherwise, nonspecific.

I would like to reassure you that there is no evidence of TB or any other serious disease on the spine MRI.

Mild disc bulges with nerve compression are noted. This would get better with physiotherapy and neck exercises.

For nerve related pain, pregabalin capsules may be used.

Dear Doctor,

I understand and thank you very much. However, her lower leg pain is not getting better in fact it is getting worse each day. Now she cannot walk because it is painful and when steps on her feet. We took some paracetamol pills but are also afraid it might have an impact on the fetus. The pregabalin capsule you suggested can this be taken during pregnancy? Is it an A or B listed drug on the FDA?
Should we take another MRI on her lower legs or will that be useless?
What kind of doctor should we go get looked at?
We are going to Seoul, South Korea in a few days.

You should get her examined by a neurologist for further evaluation.

Another MRI for legs would not yield any more useful information, as the spine MRI has covered the nerves of legs as well.

If needed, nerve conduction studies may be done after seeing the neurologist.

Pregabalin is Category C for pregnancy and may not be safe for fetus, however, confirmed data on this is lacking.
