Question: I have bloating and intestinal gas and am having a problem going to the bathroom..... i took pectobysmol and gaviscom.... today for the first time i took gasX extra strength but it hasn't helped..... i have had it off and on since mid april...... i went to see a gastroentorologist who told me to take probionics and not to eat raw fruit or vegetables..... i have pills of ciproflaxn 500mg left over from what a dr prescribed in december of 2016

Most likely, the problem you are having is called Dyspepsia.

Dyspepsia, in layman's language is known as indigestion. It is a term that describes discomfort or pain in the upper abdomen. However, it's not a disease in itself. Dyspepsia is a group of symptoms which often include bloating, nausea and belching flatulence etc.

Swallowing air while you eat or drink can cause air to collect in the digestive tract.Second, as you digest food, digestive gases are formed as a result of fermentation and digestion of food which can also collect. Either method can cause flatulence.The pathophysiology of gas and flatulence is complicated. Understanding gut microflora, gas production, intestinal transit, intestinal propulsion of gas, and sensory function within the GI tract are essential for understanding symptom generation.
The term gut microflora or microbiota means hundreds of species of bacteria which lives in our intestinal tract.These bacteria's and yeast are responsible for proper digestion and fermentation of food. Hundered of species of bacteria and yeast are found in colon of human intestine.In the same way thousands of these species flourishes in our gut these bacteria play a vital role in digestion and absorption of food.Even minor disturbances in gut microflora can lead to significant changes in gut function, including gas production.

If this microbiota is disturbed food is improperly fermented causing production of large amount of gases in gut leading to severe flatulence bloating and belching.Hence most common cause in most of the cases is disturbed gut flora.In old patient mobility of intestines is severely compromised leading to accumulation of large amount of gases in intestines.As peristaltic moments are week gas accumulates in intestines.Severe constipation or incomplete evacuation can also hinder the passage of gases.


-Small Intestinal Bacterial Overgrowth can be a reason of severe flatulence .
Small intestinal bacterial overgrowth (SIBO) refers to a condition in which abnormally large numbers of bacteria are present in the small intestine, and the types of bacteria in the small intestine resemble more the bacteria of the colon than the small intestines causing improper and disturbed food digestion.


Tests suggested-
Small bowel imaging and a gastric-emptying scan.
Rx
Take a proton pump inhibitor like Esomeprazole 40 mg twice daily. This will provide you symptomatic relief from symptoms of dyspepsia (you may need to obtain prescription for this from your local physician).
Antacid like Pepto bismol can be taken for few days along with it.
Any over counter probiotic(VSL#3) should be taken for few weeks to provide relief in symptoms of indigestion.Probiotics restore good flora of intestine and helps in proper food digestion.

Apart from this, some dietary restrictions are to be followed-

- Avoid alcohol or smoking.
- No aerated beverages.
- Take light non spicy easily digestible food for few days.
- No Fast food like pizza or burger should be taken.
- Drink a cup of cold milk daily.
- Avoid fatty foods.
-Avoid fibers for few weeks.

Bloating and acid can be caused by any kind of food. In most of the cases, it's fatty foods which cause these symptoms. Any food whether it's fat or protein or complex carbohydrate when can not be properly digested, releases large amount of gases like hydrogen, methane, carbon dioxide which causes burping.
