Suggest Treatment For Abdominal Pain And Gas Post Gall Bladder Removal

Question: I had gallbladder remived in Nov 2013. Ever since i gad abdominal issues. I was diagnosed with mild gastritis and duodenitis. In the last two months my liver enzymes went up to AST 47 Alt 92. After 15 days they were ASt 57 Alt 218. My Dr says its borderline XXXXXXX eventhough my CT scan shows normal liver ( no note about fat). He has prescribed Dexilant 60 mg daily and Carafate 2 X day. Ever since i had more upper right abdominal pain and burning, my urine was darker and frequent diarrea. Are these medications hurting my liver? Also I have losr 12 pounds in past month and a half from being terrified to Eat Because of liver condition. I am now 145 pounds and 5' 8''. My cholosterol is great and glucose Within range. What can i do to calm my liver? Why is it acting up?

I have gone through your medical history carefully and can say as following:
1. You are saying to have your gallbladder removed in Nov 2013, but ever since you are having gas abdominal issues.

You are also mentioning that you had ct-scan that showed normal liver. Can you please provide the full report of ct-scan so I can analyze the gallbladder pathways in the liver? I need to know if there is any inflammation or sludge there?

2. You are also reporting that your transaminases keep increasing. Apart post-gallbladder removal issues, hepatitis (inflammation/infection) of liver is also to be considered.

For this, I'd advise to run hepatitis testing (hep. B and C).
3. Furthermore, you are also revealing to be using Dexilant for mild gastritis and duodenitis. Indeed, there is a rare side effect of Dexilant by increasing liver enzymes. However, before running into this conclusion, I'd want to make sure you are not having any problem with hepatitis neither with gallbladder pathways in the liver.

4. With regards to darker urine, that is showing a highly concentrated urine due to losing fluids from:
- diarrhea
- non eating/drinking properly as you fear liver problems.

5. I advise to:
- increase fluids intake
- take silymarin and vit.E as liver antioxidants to help its detox
- avoid fatty/oily/fried foods that would aggravate the pain
- have your healthy meals regularly without fearing liver (if you do the right things, there is nothing to fear of).

I want to add that when it was first reported that I have increase liver enzymes (April15, 2015), I was not using Dexilant, I was on birth control pills (seosonique) and taking DGL Licorice for my stomach for month, and started taking liquid antacids for my abdominal issues. I stopped all immediately. Before the next test on May 1st, 2015, I had taken Carafate for 4 days and was under a lot of stress and stopped eating from fear of my results. The next results came in even higher as you will see from my records. I am attaching my blood tests from Nov 2014 to May 1st 2015. Also my CT scan results, ultrasound, and endoscopy. So now Dr gave me Carafate again and Dexilant and I am reluctant to use them because of fear that they may be hurting my liver. I have started them for 6 days and then stopped as I thought my urine was orange and that had some relation. Now I have abdominal issues again and what seems to me as liver pain. I would want to start them but how do I know they are not causing even more problems?

Also another question, the last blood test - fibrosure test, not sure how accurate is it to give diagnosis of fibrosure and beginning XXXXXXX based on only blood test and while US and CT show noramal liver. I am confused by this diagnosis? I have not been given any therapy my Dr for liver.

Thank you providing more of your medical data.

With regard to fibro scan it is more accurate to determine liver fibrosis or XXXXXXX than CT scan or ultrasound.

So since you have done all the exams to determine high level of liver panel and the findings are only XXXXXXX you need to treat it and then follow up liver panel.

So I suggest to:
-Lose weight
- increase your physical activity
- start to use sylmarine
- use vit E
- Repeat liver panel after 2-4 weeks.

You are reporting too that this is not associated with dexilant use (since ALT was high before dexilant use). At this situation you can continue using dexilant.

I read too that your upper endoscopy results was inflammation of duoden, oesophagus and stomach.

This findings explain your abdominal pain. You pain is not related with liver panel and liver damage.
