My mother had been complaining about abdominal pain , along with itching . She has been suspected with HILAR cholangiocarcinoma . I am attaching all the reports . Can you please provide your views on her disease and necessary treatment .

Hi

I have gone through the reports

Since her biirubin is high i would first advise to bring her bilirubin down to less than 3 by PTBD

Once that is done her HILAR cholangiocarcinoma needs to be treated with chemotherapy with combination drug of cisplatin and gemcitabine

Once her tumour shrinks in size i would advise her to go for surgical resection

Average survival time with proper treatment is around 4 years

Thanks Dr. De .. so based on the reports can u tell me how aggressive the cancer is . I mean what is the stage of the cancer .

2. is PTBD procedure is a complex operation ?? Any high risk ?

3. On an average how many sessions of chemo will be required for shrinking the tumor .

4. As I can c you are from XXXXXXX . Any idea on how much the total treatment cost around ? Just a rough figure .

5. I was told by the doc that after putting stent ( to control jaundice ) . They also will do some operation related to blood vessel in liver ( not sure about the technical term ) which is a required before chemo .

65% patients can be cured if treated properly

Hi

She is in stage 2 of her cancer

PTBD is not a complex operation

It can be done in out patient department basis and there are no risks involved

Tota cost will be around rupees 3 lakhs

The tumour first needs to be reduced in size by 3 cycles of chemotherapy so that it is suitable for surgery

Response rate with chemotherapy and surgery is very good and 65% patients can be cured if treated properly

Hi Dr. De

Can you tell me more about 5th point that I asked ? The doc at XXXXXXX XXXXXXX XXXXXXX hospital is mentioning that they need to do liver operation as well before chemo .

Also We are trying to get admission to XXXXXXX as well however we will come to know about that in a week . Do you think waiting for a week is good for her health if the jaundice is under control ??? I mean does cancer is spreading too fast ??
As you know about cost of treatment in private and XXXXXXX hospital . Adding to it every one is suggesting us to go for XXXXXXX . Hence wanted to know if we can wait for another week ?

Her jaundice is not that high that cancer will spread fast in 1 week of time.
