Suggest Treatment For Abnormal Vaginal Bleeding While Having Thrombophilia

Question: I asked yesterday regarding spotting in between periods and was told it's normal, but I'm a worry wart as it is still happening. I had my first baby 4 months ago and after about 12 or so weeks had the non-hormonal Paragard IUD put in because I have Factor V Leiden which increases your chances of blood clots. Well I had that in about 20 days is all because I had spotting the entire time and then had a period with it which was awful. I couldn't do it. So I had it removed and started the XXXXXXX Pill since it's progesterone only. I just finished my 5th week of being on it. I've had one period after starting it. That started about December 20th and now yesterday I went to the bathroom and I'm spotting. It lasted a bit yesterday afternoon and then stopped. Then this morning after using the bathroom I'm spotting again. It's enough to wear a light tampon, I just went to change it after about 6ish hours and it was close to being full. I've had some light cramping as well. Is this normal? Obviously it's too early for another period. I'm pretty consistent with every 28-30 days. Just concerned as I've never had this before.

Brief Answer:
In first month of taking pill, irregular bleeding is normal.

Detailed Answer:
Hello XXXXXXX

I can understand your concern, particularly since you have thrombophilia. But in the first month after starting a hormonal contraceptive, irregular bleeding is not uncommon. In particular, starting the Minipill can do this as it is a lower dose of hormone (progesterone) than in a combination pill. So bleeding sooner than expected, for shorter or longer duration, and spotting, is not out of the ordinary. It should correct in the next couple of months.

If you start to have a lot of pain or the bleeding becomes copious, then do call your prescribing doctor. But otherwise I think this unusual cycle is not concerning for the 1st month or two on the minipill.

Brief Answer:
It usual takes 1-2 cycles to see how it will be for you.

Detailed Answer:
It can take a couple of cycles for your body to adjust to any contraceptive pill. However, do know that the XXXXXXX pill has a fairly high rate of causing spotting - even after your body adjusts to it. So the spotting may continue to some extent. After another month you will know where you are at with how your body does with the XXXXXXX pill. And whether you are willing to go this route.
