Brief Answer:
This could be bipolar disorder or some personality disorder

Detailed Answer:
Hello thanks for asking from HCM

I can understand your concern. The symptoms you have mentioned like violent abusive behaviour, touching genitals in public place, suspiciousness, grandiose ideations, egocentric behaviour, difficulty in hearing different opinions etc suggest some psychopathological process like Bipolar disorder with mania, Psychosis or even some personality disorder.

If his symptoms are persistent and are present since long duration then personality disorder is a possibility. If the symptoms started occurring in episodes and then became persistent then we should go for bipolar disorder. He is also showing grandiose ideas and this also favours bipolar disorder. For his abnormal behaviour he should be shown to a psychiatrist.

Medicines like mood stabilisers like Sodium Valproate, Lithium Carbonate etc can help to control the symptoms. These drugs will reduce his violent abusive behaviour and are safe to use. Please consult a psychiatrist for opinion. Don't remain distressed his symptoms will reduce with medicines.

Thanks, Take care
