Brief Answer:
Cystic acne most likely.

Detailed Answer:
Hi

Thank you for asking Health care magic forum.

I have gone through your query and picture. Your problem is most likely cystic acne. This happen when acne is infection and go deeper into skin. Initially you can try applying over the counter benzoyl peroxide cream and mild hot fomentation. Another over the counter available combination gel like benzoyl peroxide and aloe vera which is more helpful also. Aloe vera in that has antibacterial property.

If you were my patient with such condition and not relieving by above measures then i would recommend local antibiotic creams like clindamycin and oral antibiotic like azithromycin.

Hope this may help you. Let me know if anything not clear.
Thanks.
