Suggest Treatment For Acute Pain While Urinating And Urinary Incontinence?

Question: Hi
Im in need of some advice im in a lot of pain i believe i have a UTI as its burning when i pass urine my tummy and bck is very very tender especially down below.
I have 16 hrs before my gps open what can i do to help ease pain ive drank plenty today 8 pints ive taken paracetamol and codiene but nothing had helped
Im also leaking urine more than normal im feeling very sick and shakey i try ro pass urine but very little comes away.
I suffer from
Polycystic ovarian syndrome
Chronic pain syndrome
Chronic pain
Fibromyalgia
Urine leaking small amount
Asthma
Depression
Anxiety
Social phobia
My current medications are:
Pregabalin 50mg twice a day
Pregabalin 150 mg twoce a day
Fluoextine 20mg once daily
Paracetamol 500mg two tablets four tomes a day
Codiene 60mg four times a day
Ranitidine 150mg one twice a day
Fultium d3 800 one twice a day
Toviaz 4mg one a day
Ventolin inhaler when needed
Seretide evo inhaler
Phenergan 25 mg one at night when needed.
Prochlorperazine 5 mg when needed

I appreciate your concern.

The medications that you use are in quite excess i.e poly pharmacy can have its toll on the functioning of the kidney. Toviaz is used for an over active bladder and can lead to urinary retention and subsequently UTI.

Please take an anti spasmodic like Buscopan plus ( Hyoscine butylbromide) or dratoverine. Cranberry juice extracts like cranmax can also relieve the pain and burning symptoms.

The only way to confirm the UTI is to get a urinalysis and urine culture , after the culture you may start empirically ciprofloxacin without waiting for the results of the culture.

I would also advise Renal Function tests, Liver function Tests and an Ultrasound abdomen done in light of the medications that you use and your current history

Hi i saw my gp today who has prescribed ciprofloxcin for a water infection and codoene for pain relief but so far the codiene isnt doing a thing its easing a little but the pain in my back is really bad im in tears from the pain what can i do to help ease it it ?

I am sorry to hear about the pain you are having

You may take Diclofenac sodium 100 mg and an anti spasmodic eg Buscopan plus to see for improvement.

If the pain persists or worsen then you may require IV medication to control or alleviate the pain by going to an ER near you.

Im not allowed to take diclofanc due to side effects however i do have tramadol here one gp is happy for me to take it but one said i shouldnt tale ot eith fluoextine ? Is there any truth in this ?
Would mebeverine help as got that but not buscopan ?

Yes you may use mebeverine.
Both dicyclomine and hyoscyamine are antispasmodic due to their anticholinergic action. Both of these drugs have general side effects and can worsen gastroesophageal reflux disease.[3]

Mebeverine is a muscolotropic spasmolytic with a strong and selective action on the smooth muscle spasm of the gastrointestinal tract, particularly of the colon. It does not have the acetylecholine side effect commonly seen in an anticholinergic antispasmodic.

yes tramadol and fluoxetine can raise serotonin levels dangerously so its not a good idea to use them together.

Instead of diclofenac you can use Naproxen sodium or Piroxicam or Ibuprofen

Bamboo shoots have been used for gastrointestinal and antispasmodic symptoms. Anisotropine, Atropine, Cindinium Cap are also the most commonly used modern antispasmodics

How long would it take for antibiotics to kick in as im still very poorly my back tummy is very sore im exhausted and keep feeling like i need to pass urine i think i pushed my body too much today as i webt to work !!!! Ive upped my fluid intake as well im also running a tenp 38.5 should tomrrow i feel a difference

Antibiotics require 48 hours at least to kick in. The frequent urination , urgency, burning micturition, are all classical symptoms of a Urinary tract infection.
Cranberry juice extracts can lead to increased alkalinity of the urine aiding in eradication of the infection, so I would recommend that in addition.
