Suggest Treatment For Adenoma In Pituitary Gland Causing Vision Deterioration

Question: 72 years male , he did heart surgery "stent" 10 years ago during preparing the stent procedure accidentally discovered macro adenoma in the pituitary gland about 9*10 mm extended intrasellar , suprasellar , and stretching optic chasm leading to pressure atrophy more in the right side , so macro adenoectomy was done into 2stages first approach via cranium , second one year later via Nose subcomplete excision of adenoectomy till reach only 2*2 mm
Now pt c/o from diminution of vision rt. eye cf 30 cm LT eye 2/60
I am asking is there any advice for this case or intervention medically minimizing the adenoma or neuroprotective he can. Use

Usually surgery is necessary.

Detailed Answer:
I read your question carefully and I understand your concern.

Looking at that MRI report it is obvious that the adenoma has grown in dimensions and is responsible for his vision deterioration.

Medication depends to a certain point on the type of adenoma. Generally surgery is s necessary. One exception would be an adenoma which produces prolactin, that type responds well to a certain medication. But to say whether that is the case though hormonal tests would be necessary. I suspect that they must have been done prior to past surgeries and it's not that type of adenoma.

In other types as I said surgery is necessary unfortunately, there is no medication to reverse or to stop the growth. At times radiation therapy may be considered as an adjunct to surgery, or in those cases where surgery is not possible or refused. Again to assess chances of radiotherapy surgery the type of adenoma must be determined, as different types respond in different degrees.
