Question: Dear Sir,
My husband is having alcohol from 12 years. When i got married he came home and fight with me on various family reasons, slowly i found that he behaved like an abnormal person. He get violent on me but now our problem increased. His behaviour is violent to all other family members. he has lost her sense . and in evening he search reasons to go out of the home. He is in real estate by profession. He found many reasons for drinking, sometime family issue, seasons, some professional parties.
He started shouting, behaving violently after coming home. Sometime he drinks at evening also.
As per him he is not a drinker. he drinks only to make his mood light. But after drinking 2 pags only he lost his sense and started behaving abnormal.
When i ask him to consult a doctor , he got more furious and behave like a mad person on me.
He never like to have drink at home. he drink at his office or a lonely place. he even sometime drink without his friends.
Most of the time he drink with his group of Dealers.
What can i do for him or my family? His age is now 39 .

Please suggest some medicine doctor , which i can provide him without his knowledge and he stop drinking.

Brief Answer:
Hi

Detailed Answer:
Hello,
Thanks for your query.
I appreciate your efforts for medical consultation for husband in so much distress.

Important aspects of your query are:
•     39 year old male
•     Known case of alcohol use for 12 years
•     Complaints of: irritability and violence after drinking alcohol.

In his case it is important to know frequency of alcohol use (daily or 1/3/5 times in a week), amount of alcohol use and nature (whiskey/vodka/beer) of alcohol use.

Many person become dis-inhibited after drinking alcohol, your husband may have such issues with him. In any de-addiction program patient involvement is necessary. If patient is not motivated, then motivation enhancement sessions are taken by psychologist after involving patient with help of elder family members. You can also take help of persons to whom patient is receptive to bring him in treatment.

In allopathy forceful treatment or treatment without patient knowledge is not advocated. For de addiction following steps are followed:

-Motivational interviewing: Indicated in cases where patient motivation is poor (like in your husband case). Some investigations are also advised specially liver function test. If there is some abnormal results feedback is given to patient, which many times enhance patient motivation to quit alcohol.

-Relapse prevention: by anti craving treatment with medicines like Acamprosate and Baclofen.

-Maintenance treatment: In this phase, some deterrent medicine (like Disulfiram) is given to patient after his consent, which causes adverse reaction on alcohol intake. It prevent patient from using alcohol due to fear of reaction.

I hope this information has been both informative and helpful for you. In case of any doubt, I will be available for follow ups.

Wish you good health.

Detailed Answer:
Hello again,
Thanks for follow up.

6 days a week use of high alcohol content liquor (whiskey/vodka) indicate alcohol dependance.

Medicines for alcohol addiction are prescription medicines and should not be given without written prescription from doctor after examination of patient.

Following Lab Tests will be helpful in his case:
-Liver function test (billirubin, SGOT, SGPT, Alk Phosphatase and GGT): Most important test.
-Other tests: Complete blood counts, Fasting Blood sugar level, Renal function test and Lipid profile.

Wishing him good health.
Regards
