Suggest Treatment For Ankle Swelling While Having Enlarged Prostate

Question : I am XXXX XXXXXXX 45years old male person.I am suffering from hypertension and under treatment of a cardiologist for last 9 years.i am tachycardic as well.at present I am taking sartel-40(half)and concorcor2.5(half)daily.i am taking flotral 10(one daily) as per advise of an urologist for last one year as it was found that my prostate is slightly enlarged.today I found a swelling in my right ankle(as appears in the attached picture).but no such swelling is found in my left ankle.for last five days I was in a tour to Darjeeling an extensive walking was there.frequency of urine is normal.i am sending previous reports to Dr XXXXXXX Bhave,Nephrologist for his consideration.I would like to ask him the possible cause of this swelling and it's treatment.

Thanks for the query

In order to help you better I need a few more details from you .

1. I see that the ankle is slightly red, it is painful as well? Is there a slight local rise of temperature when you touch it?
2. When you say you have been undertreated by your cardiologist, in what form did you feel that you received under treatment?
3. Please attach your ECHO report if it was done in the recent past.

Now there are three likely possiblities in your case
1. Swelling due to renal origin
2. Swelling due to cardiac origin
3. Swelling due to local inflammation

Going by your creat and urinary albumin creat ratio it is unlikely to be due to renal origin.
Cardiac origin swellings are usually bilateral and do not involve just one limb

Going by the history of your travel and the redness over the ankle, it is most likely that you have local inflammation. Please get back to me with the answers to the questions i have asked for more clarity

1.No pain is there.l did not feel raised temparature.

2.I wanted to mean that I am receiving treatment from a cardiologist for last 9 years.I went to him for treatment of hypertension and sinus tachycardia and I am taking medicine as per his advice.he did echo two times,the first one in the year 2009 and the second one in the year 2014.i am sending both reports for your consideration.

3. Today I have performed following tests:

a)serum creatinine
b)albumine-creatinine ratio
c)HbA1c
d)routine urine exam
I am attaching all these reports for your consideration.

Thanks for attaching the reports, I have gone through them and I am happy to inform you that they are within normal limits. The swelling of the ankle is neither due to kidneys nor due to the heart. It must be local inflammation which most often subsides on its own when you give ample rest to the area. I suggest you to apply cold packs over it and rise the leg end while sleeping. If the swelling does not subside then please approach an orthopedician for further evaluation.

I want to ask another question to you.I tested albumin creatinine ratio in February,2014 from two different labs and these values were 4.6 and 1.2.but yesterday this result came as 14.8. though it is within normal range but it has been enhanced during this period.My questions are :

1.does it indicate any problem in kidney function?
2.why such enhancement occurs ?
3.Is that enhancement irreversible?
4.what measures are to be taken to prevent such enhancement?

as per advice of my cardiologist I have taken Rosuvas 5 for last 6 months.at present it has been stopped by my cardiologist.

Albumin loss in urine

Detailed Answer:
Hello

Thanks for getting back

Protein loss in urine is multifactorial. It can vary just like our blood pressure.
The common causes for albumin loss in urine are
1. Exercise
2. Starvation
3. high protein diet
4. Fever

The above issues lead to albumin loss however it will be within normal limits. There is no need to worry about it.
The question of retarding the progression of protein loss only occurs when it is beyond the normal limits.
Now to answer your question one by one

1. It does not indicate kidney problem
2. I have answered this
3. It is reversible with certain drugs but it is only required in pathological states
4. Adequate control of blood pressure and a low salt diet
