I can certainly understand your concern and have worked through your attached query in detail.
As per your symptoms you seem to have excessive preoccupation with your health and undue fear. This is causing marked distress and Anxiety.
Heart attack and stroke are extremely unlikely for your age. You can get your blood pressure monitored and an ECG done to rule these out.
Your current symptoms are suggestive of tonsillar Infection. This require tonsillectomy ( surgical removal of tonsills) for effective cure.
HIV is a sexually transmitted infection and is not the case in you.
I generally prescribe a short course of antibiotics to combat Infection and a longer courses of oral anxiolytic Medication to treat Anxiety in my patients with such symptoms.
Please check with your physician if he shares my view and if can prescribe these medications to improve your symptoms.

Symptoms in the tonsills are related to Infection and require antibiotic therapy and subsequent tonsillectomy.
Tonsill enlargement can be unilateral or bilateral.
Right ankle and heel pain require oral analgesics.
Consultation with a psychiatrist and initiation of anti-anxiety medications should take place at the earliest.
