                                    Brief Answer:
You should see a nephrolgist immediately
Detailed Answer:
Hello
Thanks for your reply
This can be caused by obstructive nephropathy or connective tissue disease like gout or an auto immune disease.
I would advise you for serum uric acid and barium study of urogenital tract.
Ask him to quit smoking.
Here I would like to know number of cigarettes he smokes a day and since how long?
Elevated Blood urea and creatinine is a matter of concern this could be dangerous if not treated in time.
It also warrants physical examination, so in my opinion you should immediately see a nephrologist as earliest.
Thanks for your question
best wishes
