Brief Answer:
Please upload more information and rest of the reports

Detailed Answer:
Hello
I appreciate your concern
Looking at your description and reports this could be a case of kidney disease with polycythemia.

I would like further information to help you better
Relevant medical history like diabetes or kidney disease in past
Any complaints of bleeding in urine

I would advise you to get a urine test for microscopy as well as culture.
Please upload me rest of the reports, this will help me to reach at a conclusive diagnosis

In my opinion you should immediately consult a urologist as earliest possible.

Please revert back to me with my answers , I will definitely help you in best possible way.

Brief Answer:
You should see a nephrolgist immediately

Detailed Answer:
Hello
Thanks for your reply
This can be caused by obstructive nephropathy or connective tissue disease like gout or an auto immune disease.

I would advise you for serum uric acid and barium study of urogenital tract.
Ask him to quit smoking.
Here I would like to know number of cigarettes he smokes a day and since how long?

Elevated Blood urea and creatinine is a matter of concern this could be dangerous if not treated in time.
It also warrants physical examination, so in my opinion you should immediately see a nephrologist as earliest.

Thanks for your question
best wishes
