Question: My mother is 75+. Has Atrial fibrillation and is under Pradaxa 110. She was taking Cilacar 10 and Losar 50 both for last 6 months. Recently the BP fallen to 90/60. Both Cilacar and Losar has been withdrawn. Today the BP came back to 130/80. My question
1) Are CILACAR and LOSAR 50 both too much?
2) Withdrawal of both advisable?

I have gone through your question and understand your concern.
Let me answer your question one by one.

1. Cilacar is a calcium channel blocker whereas losar is angiotensin receptor blocker. Both have different mechanisms of action, less side effects and can control hypertension very well. As your mother had a history of hypotension, both might not be necessary. You can try with any of the above medicines with regular monitoring of blood pressure. If it remains in normal range, single drug treatment can be continued. Otherwise, you might return back to combination therapy.

2. Withdrawal of antihypertensive drugs in a known hypertensive patient is not advisable. Withdrawal becomes more risky as she was under a combination therapy. So a single drug may be withdrawn and monitored. But complete withdrawal of therapy is not very practical.
