Suggest Treatment For Avascular Necrosis Of The Femoral Head?

Question: I'm a patient of ANFH (Avascular Necrosis of Femur Head) in China. My problem is caused by the use of Glucocorticoid for the cure of Acute Myelitis in February,2013. I'm in the stage of I or IIA. I haven't felt the pain of my hip joint till now. There is no limitation of the movement of my hip joint, but MRI showes clearly that there is the lesion, and my femoral head hasn't collapsed.
I have two questions:
1、What is the effective treatment for ANFH in early stage?
2、Some doctors said that High Energy Shock Wave (Extracorporeal Shock Wave Therapy) is very effective for my condition, does this treatment really work?

In early asymptomatic avascular necrosis of femoral head low-frequency pulsed electric and magnetic fields can stop the further disease progression.
Free vascularized fibular grafting significantly alters disease progression in precollapse lesions and is even useful in modifying disease in mildly collapsed and early arthritic hips.
You can start with Extracorporeal shock wave therapy and see the improvement with serial MRI scans. In case there is any further deterioration then do consider going for surgical treatment.

Thanks for your answer.
I have two more further questions:
1 Does medicine work for my condition? Some doctors said I could take Calcitriol, and Fosamax Plus or Salmon Calcitonin Acetate, but some said they wouldn't work.
2 I have seen some of the best Orthopaedic Surgeons in China, and I want to know which hospitals are the best at Orthopedics in the U.S.? Can you name some of their best doctors for this disease? What are their typical treatments for early ANFH? Can foreigners get the best medical care in the U.S.?

1. Medicines will not help you much. Taking calcium, calcitriol or fosamax plus will not reverse the disease process.
