Hello XXXX, Welcome to HCM..

I've gone through your query and understand your concern..

Now your list of medications include XARELTO a blood thinner , generally a novel one but still it can cause black stool..

Blood is a irritant for our gut , so it can cause diarrhoea too..

Weakness can be there due to drop in hemoglobin as well as dehydration and disturbance in Acid-base balance of body..

Firstly, you need to stop XARELTO for the time-being, seek help so that you can get in to your nearest ER/ Urgent Care at the earliest..

Please don't delay any more, as you need to be thoroughly investigated for Coagulation profile, liver and kidney function as well as Hemogram and EKG and Chest X-ray, and / UGI-Endoscopy later on..
