Suggest Treatment For Bloating, Belching And Pain In Stomach

Question: For the last 3 years I have been having gastric problems which include intense bloating, belching and passing of gas with pain in my stomach and abdominal areas in random. Initially I used to take ranitidine 300mg or pantaprazole 40 or rabeprazole 40. But then I consulted a doctor after symptoms increased with frequent intestinal disorders accompanied by sleeping problems with increased heartbeat and regular vomiting after 2 hours of dinner. This vomiting was giving me temporary relief. In tests it was found that I had a slightly fatty liver with increased spleen and slight gastric sign. I was prescribed with Pantaprazole and Levosulpiride 25 mg with alprax 0.25 per day for 1 month. After 1 month my problems were cured by 60%. But for the last 1 year, I have taken pantoprazole and levosulpiride daily with low fat diet (as meat and milk and junk food causes too much indigestion for me). But still I have the bloating and belching and gas problems.

Point to mention: I used to be a regular alcoholic but quit alcohol for last 2 years. My regular diet includes boiled rice, vegetable curry, fish and occasionally chicken. I have been in high depression and mental tension for the last 1 year due to career and love related issues. But no problem with sleeping.

As you mentioned that ,now a days you are taking pantoprazole and levosulpiride , this is a right treatment for your problem .
You are also taking alprax0.25 mg for sleep .
The depression , anxiety , tension are few predisposing factors responsible for your disease .
However , fatty liver may be due to alcohol which you were using 2 years back .
Following measures may help you :
Try raising the head of your bed about 4 inches with blocks. It also might help to avoid eating or drinking for 2-3 hours before you lie down. To control stomach acid one should not drink alcohol ( as you quit it some time back ) or drinks with caffeine in them or eat chocolates , fish chicken fried food or spicy or greasy foods.
Avoid eating as this is most common cause of bloating , belching and gas formation .
Since you are taking pantoprazole 40 mg . When such patient visit , I prescribe them to take pantoprazole-D + levosulpiride .
Overeating is the another factor responsible for such symptoms .
Depression is another reason of such symptoms.
I prefer clonazepam 0.25 mg in the night as it will relieve anxiety, tension & help in sleep .
As you mentioned that vomiting relieve symptoms . When you vomits than stomach emptied so patient feel relief as some hydrochloric acid also come out.

But sir, even after taking daily pan+levosulpiride, and following diet charts, I am not getting desired results. Every night I have to empty my stomach, before going to bed.

Still need not to worry ,when such patients visit I advise them to take some antacid ( if contains oxetacaine , better) +enzyme ,immediately after meal.
Pantaprozole as advised ,but instead of levosulpiride I recommend "REBAMIPIDE" 100 mg 2-2 on empty stomach for 3-6 months and results are nice .
But once again I would like to say ,overeating , tension , anxiety and worries are main reason of such bloating ,belching and gas formation , so be careful about these factors .
