Question : I've had a bloated stomach for 3 weeks. I do have done constipation but other than that no real discomfort. My dr did a CMP and all levels were normal. I also had an ultrasound of my liver and gallbladder and it showed I have fatty liver. Can this be causing my bloating ? I'm so worried that it's something we're missing.

I forgot to mention that I do have irritable bowel but have never had my stomach stay bloated this long. Also have experienced horrible itching in the anal area for past year but only at night.

Hello, greetings! Thanks for posting your query in hcm.
I have made a note of your symptoms, feeling of a bloated stomach is a common problem associated with irritable bowel syndrome. This is due to the erratic movement of food in the intestine.

Your Comprehensive Metabolic Panel is normal. That means, the chemical and metabolic processes in your body, electrolytes, blood sugar levels, cholesterol, liver and kidney functions are in the normal range. If you have a copy of the report, I will like to review them. There is an option to upload reports available on this page.

Fatty liver i.e., accumulation of excess fat in the liver rarely causes symptoms like bloating, unless it is accompanied with inflammation, a condition called steatohepatitis.

You can reduce the bloating in your stomach by the folllowing factors:-

1. Limit the consumption of high fiber and fatty foods. Avoid beans and lentils.
2. Avoid fizzy drinks, do not talk while eating, chew well.
3. Eat regular small meals, take measures to reduce constipation.
4. Consume probiotics. These can be obtained by consuming yoghurt or are available over the counter.
5. Limit intake of processed food.
6. Avoid artificial sweeteners as they contains certain compounds that can mess up with your digestive system.
Continue with Nexium and try the above measures for 3 weeks. If you still experience the bloating sensation, I will then refer you to a gastroenterologist.

Itching in the anal area, especially during the night points towards infestation in the intestine with a parasite called pinworm, or Enterobius Vermicularis. The female worm is more active at night and comes out of the anal area to lay eggs, thus causing itching. I would suggest my parents antihelmintic drugs like albendazole 400mg one single dose or mebendazole 100mg. Both these drugs have to be taken orally and have to be repeated after 2 weeks. Additionally, I would also a prescribe an antihistaminie drug like allegra in the night to counteract the itching. You can obtain a prescription for these drugs at your local physician.

Ensure anal hygiene, keep the area dry and avoid wearing tight fitting undergarments.

Antihelminthic and antihistamine may take a week or two to be effective. If you don't get better in a couple of week, I will order further evaluation and follow up for a fungal infection/ scabies infestation.
