default
Posted on Wed, 1 Oct 2014
Question: I have white stringy tissue in urine. My last several urine specimens have had microscopic blood in them. I have had bacteria but they culture them and always say that it's not the kind of bacteria that they usually treat but they gave me cipro anyway and I have finished that. Not there is white stringy things in my urine with a foul smell and I have had pain on my fright side just under rib cage in front and the pain radiate through to my back and that pain has been on and off for several months. Any ideas what is going on? The urologist doesn't think that it's kidney related and I am scheduled to have a scope in my bladder next week but the white stringy stuff in my urine is a new symptom and I am scared. Any ideas what is causing the white stringy stuff in my urine? Please help.

explained

Detailed Answer:
Hello and Welcome

I appreciate your concern.

The white stringy tissue could be due to

◦Drugs eg – Propofol
Medical conditions – Chyluria which is due to an infection in urinary pathway,
in filariasis (complicated stage),lymph flow abnormality will lead to white milky urine,
pyuria,
phosphate crystals,

work up to rule out urolithisis must be done , as hematuria too is present
an ultra sound to rule out any gall bladder pathology should also be done

Oral therapy with an empirically chosen antibiotic that is effective against gram-negative aerobic coliform bacteria (eg, Escherichia coli) is the principal treatment intervention in patients with cystitis. The first-choice agents for treatment of uncomplicated acute cystitis in women include the following:

Nitrofurantoin monohydrate/macrocrystals

•Trimethoprim-sulfamethoxazole (TMP-SMX)

•Fosfomycin

The fact that you have not been able to clear the UTI also constitutes an indication to look for any anatomical abnormality or cause

Hospital admission may be indicated for some patients with complicated UTI. Complicating factors include the following:

•Structural abnormalities (eg, calculi, tract anomalies, indwelling catheter, obstruction)

•Metabolic disease (eg, diabetes, renal insufficiency)

•Impaired host defenses (eg, HIV infection, current chemotherapy, underlying active cancer)

Behavior modification, such as good oral fluid intake to enhance diuresis and frequent voiding (including post intercourse voiding) may be helpful in reducing recurrent infection Adequate fluid resuscitation restores effective circulating volume and generous urinary volumes. Antipyretic pain medications may be administered, as appropriate.

Cranberry juice extracts to increase alkalinity of urine, anti spasmodic like buscopan plus for symptomatic relief may also be used

wishing you best of health
