Question : I had a small boil on the lower part of the shaft of my penis. It has now become like this. Not much itching. Is this a hair break?

I have reviewed the attached photograph.
It seems to be some infection of the skin which is now healing.
It can be due to hair break.
If I were your treating doctor I would recommend you to apply antibiotic cream like neosporin on the affected area for 5-7 days till the lesion resolves.

Hi Doctor

I hadn't notice earlier but the primary lesion is on the body and not on the shaft. Also last night a new smaller lesion appeared on the shaft next to the one I showed yesterday afternoon.

I have attached 3 images
1) image from last night shows the 3 lesions
2) image from this morning after having applied the ointment over night
3) the ointment I'm using. Neosporin was not available anywhere.

My questions are:
1) I hope this is not syphillis
2) is the ointment good or should I use something else based on this morning's image
3) how often do I need to clean the area & apply the ointment
4) I feel puffiness and slight pain around the primary lesion is that ok. No itching so far.
4) how contagious is this. I'm asking from the perspective of how I can avoid giving this infection to someone else

It is not syphilis, it is folliculitis

Detailed Answer:
I have reviewed the attached photographs.
I can see 3 lesions which are showing the presence of crusts.
These are not due to syphilis. In syphilis an ulcer is formed , mostly single.
These are probably due to infection of the hair follicle.
Betadine ointment is not very effective. I would recommend you to apply either mupirocin or fusidic acid cream on the affected areas.
Wash the area with plain water and apply the cream twice daily.
Continue this for 5-7 days till the lesions heal.
Slight pain and puffiness is normal to happen with folliculitis.
You need not worry as this is not contagious but you should not go for unprotected sex till it heals.

I also see a fresh boil above the primary lesion.
I've have started the fucidic acid antibiotic cream since last night. I will continue with the cream just wanted your opinion on the healing.

The latest attached image shows few new smaller boils.
The lymph node where the cartoon finger and thumb are pointing to is swollen. Feels like a small ball inside. It's causing discomfort/slight pain in walking and peeing.
I am also feeling a soft pain all over th body just like in viral fever but without any fever

The new lesions that have appeared are in the form of fluid filled blisters or solid bumps?
If they are blisters then you have developed herpesvirus infection and all the symptoms are because of that.

Start with antiviral drugs, herpes is contagious

Detailed Answer:
The new lesions look like a cluster of small blisters grouped together.
Your symptoms along with the appearance of these suggests high possibility of herpetic infection.
Continue with the application of fusidic acid cream on the affected areas twice daily.
In such cases I recommend my patients to take antiviral tablet like tablet aciclovir 400 mg thrice daily for 1 week along with paracetamol tablet sos for pain and body discomfort.
Aciclovir is a prescription drug and you need to see the doctor in person to get it.
Herpes is contagious. It is transmitted sexually. You should avoid intercourse till the lesions heal completely.

Thank you doctor. i went to a hospital today and the doc said it looks like herpes to her as well. I have attached my test reports. Kindly review and advise.
I'm not sure which one of the tests was for herpes though.
She has advised
1) Acivir 400 DT 3x 7 days
2) Azithrol 1x 3 days
Can i take crocin when i feel body ache and fever?
She has however asked me to stop using the fusidin acid cream.

All tests show negative what does that mean. Could it be some other type of infection.

Tests for HIV, syphilis, hepatitis B & C are negative, can take crocin

Yes you can take crocin for bodyaches and fever.
I have reviewed the report.
She has basically tested you for other sexually transmitted diseases which are associated with herpes genitalis.
There is no test advised for herpes infection.
It is good that all these are negative. She has tested you for HIV infection, syphilis, hepatitis B and hepatitis C infection.
You should take aciclovir for a week and azithromycin for 3 days and revert back after 1 week. Fusidic acid is basically to prevent secondary bacterial infection in these lesions.

My concern is how can I stop this from spreading. The puss from the blisters is causing new blisters. I'm cleaning it 4 times a day to clean the dried puss that spreads on other areas.

Can I use some dressing for the area. I'll do it myself. And change frequently. Or should I keep the area as open as possible.

I have attached an image. Im feeling better. The soreness if the area has decreased and so has the swelling of the lymph node. But I have a lot of pain in my balls. Should I take crocin?
Also How sure are you that this is herpes. :(
What changes will j have to make in my lifestyle. I've read horrible stories about herpes virus.

Yes take crocin, for confirmation go for ELISA test for herpes infection

Yes you can take crocin tablet for pain in the testicles.
I am quite sure that the clinical presentation is of herpes infection but to be 100 percent sure you can go for a blood test.
The name of the test is ELISA for hsv1 IgG, hsv1 IgM, hsv2 IgG and hsv2 IgM.
Don't take too much stress about this infection as it is quite common and not very dangerous.
The first episode is the most severe. Recurrences become less severe.
Try to go for protected intercourse especially during the blister stage as it is most contagious during this stage.
