
Suggest Treatment For Brain And Liver Metastasis After Tumor Removal From Pituitary Gland

Summary of the case history:
- Severe head ache and right eye lid started closing
- MRI report said there might a tumor and suggested us to consult the doctor
- Doctor detected that there is a tumor in pitututory gland and undergone a surgery through nasal passage.
- Post surgery, doctors advised us to go through the radation process as tumor is not completely reomved in the surgery. ( completed 8 sitting now in radation)
- Parallelly, hospital has sent the blocks for the testing and the result came out as metastatic and primary might be in lung.
- We did MRI in lung and results came as clear and suspected primary is in liver.
- Doctor advised us to do the biopsy in liver and we are waiting for the results to come
My queston is :
1. Are we on the right track in the treatment?
2. Is metastatic is life threatning?
3. Doctor says, Liver and brain cannot come together and it seems to be rare.

Hello dear. I have gone through the reports. I appreciate your concern. Your father is certainly not in a good condition.
Metastasis to brain and liver can occur with the primary in the following areas
Lung, Melanoma, kidney, Gastrointestinal, neuroendocrine(small cell) and breast.
So he requires following test for primary localisation
In my opinion a PET CT will be a good modality to look for primary lesion
Also endoscopy of upper gastrointestinal tract and colonoscopy will also be required if PET CT is unable to localise the primary.
Most importantly is the repeat review of the tumor block or a fresh biopsy from liver and to look for the evidence of tumor i have mentioned above as primary.
Now when primary is localised then we will treat it as we treat a metastatic tumor of above said primary. Say for example if primary is localised to kidney then it will be treated as metastatic tumor of kidney.
If we are unable to localise the primary, then it will be treated as cancer of unknown origin.
Now comes the treatment.
Radiation to brain is required to prevent seizure and tumor growth.
Chemo will be required as per the biopsy report.
Hope that makes it clear.
It is very sorry to say that survival will not be very long, but some patient do survive long so stick to the treatment.

I have one more concern in our treatment now :
Our sugery happened at Sakra hospital in XXXXXXX and radiation at Vydehi hospital.
Basis the nimans block report and MRI on lungs, sakra doctor's who did the surgery suggested us to stop the radiation in brain and basis the liver biopsy report they would like to suggest the treatment.
Meanwhile, Vydhehi doctors called me saying dont stop the radiation treatment.
I'm bit confused as im getting different opinion from two hospitals.
Currently we stopped the radiation.

Hello dear. In any case irrespective of the biopsy results, radiation to brain is must for improving quality of life. So continue with it. Absolutely no confusion over it and moreover a surgeon cannot decide the radiation.
It is good that a liver biopsy is on the way. It will help to choose the definite treatment.
