Question : Bruise on underside of dick near base and bruise on front of balls, is it possible for him to do this to himself, or did he get it from sex with someone else?

Intertigo, drug rash are common causes. STD a rare possibility.

Bruise under penis and over the scrotal sac are common with some skin conditions like intertrigo or drug eruption. It may be also due to some irritant external applications.
Very rarely a STD cause may be a reason.
With a careful examination, a Dermatologist will be able to fix the diagnosis. So consult a Dermatologist and get well soon.

he does not have a skin condition or use any drugs or lotions that would cause this. You did not answer my question - did he do this him self while masturbating or with someone else. Due to the position of the bruising I think it is much more likely that someone else gave him those bruises. He masturbates the tip of the penis, and someone facing him would have maybe a thumb or hand or tailbone in a position to give him the bruises. Please be specific about what sex position (self or with another) would cause the bruising.

It is quite unusual to have bruises during masturbation either self or with the help of others especially at the base of penis and over scrotum. If you could upload the picture more information may be able to be extended.
Forcible and vigorous rubbing and fingers with nails or rubbing against a rough surface during the act of masturbation can cause bruises. But not common at the base of penis and over scrotum.
