Question: Dear doctor, thank you all your wonderful help in this site.

BACKGROUND
I am writing to you because un monday nov 17th on afternoon (7 days ago) i had protected CASUAL anal sex (give and receive), but had unprotected oral sex (give and receive). My sexual partner (male) had 1 round flat red coloured lesion between lip and chin but said it was not herpes (i am not 100% percent sure it is true). While i was receiving anal sex i was masturbating with a condom on (which had spermicide in it).

I also took 100mg viagra pill that day before sex (i am not used to taking viagra). It is the second time in my life i have taken viagra.

Afterwards i got nervous and since i did not had a shower or bathroom nearby i applied an aromatic hand sanitizer on penis shaft and tip and also moist kleenex towel (for cleaning babies) around tip.

SYMPTOMS
Since then, 1-2 days after sexual experience i HAD this burning sensation in the tip of my penis. I didn't feel any pain, fatigue or even fever, but i HAD this persistent intermittent pain for almost 11 days and also feel quiet frequently the need to urinate. Also i think i HAD some very mild ache in testicles but i think it might have been in my mind i am not sure of this.

I am a daily marijuana smoker and i am 100% sure that every time a smoked marijuana the burning sensation was much more obvious and constant.

4 days ago (friday nov 28th) i stopped smoking marijuana and burning sensation ALMOST disappeared. I also took Advil (ibuprofen) 1 pill per day for 2 days.

Today, 2 weeks exactly after sexual experience the burning sensation is completely gone and NO symptoms persist.

ABOUT ME
Things you may need to know:
1. I am a frequent marijuana smoker (I smoke 1 regular sized joint every night after work).
2. I am HIV+ currently on successful HAART with undetectable VL and 550 CD4 and 43% CD4%.
3. I was diagnosed with HPV warts on march 2014 and they were surgically removed but site where some of the warts have stayed a bit inflamed afterwards leaving what feels like bumps in 3 areas (i went back to urologist for this and he said it was inflammation response of the skin to surgical removing of warts and i should not worry).
4. Other than that good health and good overall frequent blood lab tests (CBC, liver, kidney, etc).
5. I masturbate DAILY and sometimes did 2 times a day. (almost once a day on average).

TESTS PERFORMED after 4 days of possible exposure:
1. Urine Culture: negative results for common pathogens looked for in test (lab does limited search for parasites in this type of test).
2. Urine PCR for CHLAMYDIA and GONORRHEA: NEGATIVE.
3. Urine Test (NOT PCR) for HSV 1 and 2: still waiting for results.

TESTS PERFORMED after 10 days of possible exposure:
1. Urine Culture: negative results for common pathogens looked for in test (lab does limited search for parasites in this type of test).
2. Urine PCR for CHLAMYDIA and GONORRHEA: still waiting for results.
3. Urine Test (NOT PCR) for HSV 1 and 2: still waiting for results.
4. Mycoplasma (urine): still waiting for results.
5. Proteinuria in urine: results were normal.


CONSULTATION QUESTIONS FOR REVIEWING DOCTOR
1. Could this burning sensation for almost 11-12 days have been caused by hand sanitizer on penis + frequent masturbation + smoking marijuana daily (heavy) and LASTED THIS long due to the fact that i continued smoking marijuana daily and heavily (and every time i smoked burning was worse)?

2. Now that all symptoms are gone what would your best guess be as to what could have caused this symptoms?

3. Do you think that considering my previous symptoms that Herpes is a strong possibility?

4. Any other tests that you would recommend like for example Trichomoniasis?

5. I have also been drinking Cranberry juice for the past 3 days. Could this have HIDDEN my symptoms and now that i have stopped drinking Cranberry juice could they come back?

6. What other symptoms do you recommend to be on the look for if you think it might be and STD.

Brief Answer:
This can be STD. Less possibility of herpes. take regular treatment.

Detailed Answer:
Welcome
i gone through your history. I would like to give you following advise.
if possible quiet marijuana.
avoid sex with multiple partners.

burning sensation can be due to STD. sanitizer will not cause such symptom for prolonged time. All these symptoms are associated with sexually transmitted infection. take treatment according to your culture report.
Continue treatment for HIV and get regular laboratory check up done.
This can not be herpes as it is associated with development of pain ful vesicles. cranberry juice will help to avoid urinary infection.
this can be simply due to urinary tract infection which causes burning pain. In case of STD, there may be associated whitish discharge from penis or ulcer formation for which you have to look for.

Taking into account that my only symptoms lasted for about 10-11 days and now are completely gone, do you still think it might be an STD? If so which ones do you think COULD be the cause of my symptoms?

There was no discharge from penis at all and no lesions or sores.

Now that i have no symptoms, do you still think i may need treatment?

I mentioned some of the lab tests i took. What other tests would you recommend doing?

Do i have to wait more time for chlamydia/herpes/gonorrhea/trichomoniasis/Mycoplasma tests to be accurate? How many days for each test to be accurate?

Finally, why would smoking marijuana have made burning sensation worse?

Brief Answer:
wait for lab result. immunosuppression increases risk of infection.

Detailed Answer:

Dear, as you are already taking treatment for HIV, there are increase risk of infection due to immunosuppression. HIV itself increases the risk of other sexually transmitted diseases like chlymydia, gonorrhea etc.

If you have no other symptoms, then wait for your result to come. otherwise no need to test further. if you have no any symptom now, then you can wait for lab result depending on which you can start treatment.
marijuana aggravates immunosuppression there by increasing the risk of infection.
it also increases susceptibility of individual for urinary tract infection which might be the cause of burning in your case.
