Suggest Treatment For Chest Pain When Suffering From Acute Esophagitis

Question: Hi. I was diagnosed with acute esophagitis 1 1/2 months ago. EDG said esophagus was inflammed I have this constant chest pain behind right breast bone and my back feels sore and tender They put me on omeprazole and ranitidine 300mg. Took that for a month. Didnt do any good. T hey had to take me off omeprazole because of my bone density problem and put me on sucralfate 4 times a day a couple of weeks ago along with the ranitidine. I am still getting this constant pain. This is driving me crazy and is all I think about. Why do I get this constant pain? Thanks in advance. by the way, taking sucralfate 4 times a day is a pain

Hi XXXX Good evening. I am Dr Shareef answering your query.

The following might prove to be helpful in reducing the need for any particular drug in your case.

1) Elevate the head end of the bed with two blocks on each side, so that the regurgitation does not occur while lying down, and the contents of esophagus and stomach flow down with gravity.
2) Some modification in your diet like more of roughage, enough liquids, avoidance of oily, fried, and fast food.
3) Avoid eating from outside if you do, as intestinal infections do creep in from such food which are not always hygienic, and might be responsible for your esophagitis.
4) Regular routine exercises like morning walk depending on your medical fitness.
5) Prescription for a prokinetic agent and a proton pump inhibitor for symptomatic relief and to have a control on the acid production and facilitate movement of gastric contents downwards. This could reduce your need for sucralfate, although it is a good compound.

The pain in your chest behind the sternum could also be due the spasm of esophagus, which occurs sometimes in case of esophagitis. But to be on the safer side I would not hesitate to refer you to a cardiologist (after recording your blood pressure and blood sugar) just to rule out any cardiac problem.
