Posted on Mon, 20 Apr 2015
Question: Hi Doctor,
I am 50 years old and developed nasal choking almost a month back. Thought it was common cold and will go after its due course of a week. But, now after a month it still persists, rather now as usual breathing is difficult and choking is there but no mucus is coming out as I blow my nose. I have already done a course of antibiotice. Have also been taking Ayurvedic treatment. But no change. What do u suggest?

Brief Answer:
Medication trial as detailed followed by investigations if required.

Detailed Answer:
Hi,

Thank you for your query.

1. Get your nose examined for any nasal turbinate hypertrophy, and request a prescription for an anti-allergic, anti-leukotrine, a nasal steroid spray and steam inhalation trial for 15 days. Nasal decongestant drops (such as Otrinoz-P) can be used for a few days (short term only, 3-4 drops twice a day).

2. If the response is good, continue with this medication and taper off after a month.

3. If the response to medication is not satisfactory, get a plain CT PNS (Para Nasal Sinuses) done with a post decongestion scan after liberal use of Otrivin nasal drops (5-8 drops both sides, single use after the first scan) and after waiting 15 minutes by the clock. look for sinusitis and nasal turbinate hypertrophy.

4. The nasal valve areas must be examined. This can also be a cause at this age. A diagnostic nasal endoscopy is recommended at this stage.

5. Upload the images here for further treatment options.

I hope that I have answered your query. If you have any more questions I will be available to answer them.

Regards.
