Question : I have axial lower back pain, want ideas as to treatment? No pain in legs or upper back. Pain doesn't radiate but is situated in lower spine. I can move around, but when walking, sometimes for an hour, or sometimes for just 10 minutes, get intense pain that feels like lower spine is compressed and stiff. Pain is sharp, not dull or achey. Pain is relieved by bending over or taking weight off, lying down. Pain is about 4-6 on a scale of 10 with 10 being the worst. Over 2 years ago I had a muscle tear in my lower rhs lower back (same general area) about 2 inches from my spine from lifting and twisting stacking logs. AFter 2 weeks in bed I recovered, and after a few months I had no symptoms - I think primarily because I undertook weight training 3 times a week for a year. I have noticed that when I have ceased weight training, especially training that worked lower body and core, I have suffered relapses and scar tissue builds up at that injury site. Recently there was a lot of scar tissue, which with massage breaks it up. I'm certain that this is a result of that primary injury as there is no pain at the original site.

Brief Answer:
You are having chronic backache likely to be due to muscle weakness.

Detailed Answer:
Hi,
Thanks for choosing healthcaremagic for posting your query

I'm Dr. Sudhir Kumar,

I have gone through your question in detail and understand your concern. You are having chronic backache likely to be due to muscle weakness as it occurred after you ceased weight training exercises. You can take analgesic such as ibuprofen for pain relief. Back exercises and physiotherapy will be helpful.Calcium and multivitamin supplements are helpful. You can get your vitamin D3 level and you can take its supplements if found low.

You can get MRI of lumbar spine done to exclude degenerative disc disease and other causes of backache.

You can discuss with your treating Doctor about it.

Hope I have answered your query.
If you have any further query, I would be glad to help you.
If not, you may close the discussion and if possible you may rate the answer for me, so that I get a good feedback.
