Suggest Treatment For Chronic Back Pain

Question: My mother is suffering from back pain from more than 10 years now. We have consulted many orthopedic doctors and everyone is suggesting operation. She is able to walk and manage her daily activities by wearing belt. Kindly advice whether the operation is absolutely needed. I have attached the MRI taken in 2013 Dec.

I'm Dr Sudhir Kumar

I have gone through your query and understand your concern. Your mother is likely to be having lumbar spondylosis as it is common in her age. It can be confirmed by x ray or MRI. Your report of MRI is not available on website. Probably it has been failed to upload. You are also advised vitamin D test as it is common cause of back pain. She is likely to be benefitted by physiotherapy, analgesic such as ibuprofen, Diacerine and glucosamine and vitamin D3 if level is low. Comments about operation can be only done after seeing MRI. Kindly upload your MRI report.

She is having mild spondylolisthesis with spondylosis ( degenerative changes)
Operation is not absolutely necessary. She can manage by lumbosacral belt physiotherapy,analgesic such as ibuprofen, Diacerine and glucosamine. Vitamin D3 can be taken if found low.
If it is very painful ( backache) and not managed by these drugs then in situ fusion ( operation) is required.

Hi,

In recent days she is complaining about more pain and numbness in the legs. I have below questions.

1. Will Yoga and reducing weight will help her to solve this problem?
2. Most of the consulted doctors recommended operation and using titanium rods to fix the issue. Is this the recommended way in case we go for surgery?
3. Whom should perform this operation neuro surgeon or a orthopedic surgeon? Any recommendation in XXXXXXX please.

1.Reducing weight will help. Back exercises may help. But Yoga is unlikely to help.
2.Fixation with titanium rod ( and screw) is Okay.
3. I will recommend operation from an orthopaedic surgeon as problem is related to bones more however neurosurgeon also do this operation. I doesn't know much about doctors in XXXXXXX area.
